🦵 The Femoral Triangle — A Key Anatomical Landmark in Clinical Practice

The femoral triangle is a vital region in the anterior thigh, serving as an important gateway for neurovascular structures entering and leaving the lower limb. A precise understanding of this area is essential for clinicians, surgeons, and physiotherapists alike.

🔹 Boundaries

Superior (Base): Inguinal ligament

Medial: Adductor longus

Lateral: Sartorius

Floor: Iliopsoas (laterally) and pectineus (medially)

Roof: Fascia lata and skin

🔹 Contents (Lateral → Medial)

NAVEL:
Nerve → Artery → Vein → Empty space → Lymph nodes

🔹 Clinical Relevance

Palpation of femoral pulse

Femoral nerve block administration

Access for catheterization or venipuncture

Assessment in vascular pathologies (e.g., femoral hernia, arterial occlusion)

Facial Nerve (Cranial Nerve VII)

The facial nerve is the motor nerve of facial expression, carrying additional taste fibers (anterior 2/3 tongue via chorda tympani) and parasympathetic fibers to salivary and lacrimal glands.

After exiting the stylomastoid foramen, it enters the parotid gland, where it divides into five terminal branches that radiate across the face like a fan.
These are remembered by the mnemonic “To Zanzibar By Motor Car” — Temporal, Zygomatic, Buccal, Mandibular, and Cervical.

Branches and functional areas:

🔷Temporal branch:
Innervates frontalis and orbicularis oculi (upper part). Controls eyebrow elevation and forehead wrinkling.

🔷Zygomatic branch:
Supplies orbicularis oculi (lower fibers). Responsible for eyelid closure — important in protecting the cornea.

🔷Buccal branch:
Innervates buccinator, orbicularis oris, and muscles around the nose and upper lip. Enables smiling, blowing, and whistling.

🔷Mandibular branch:
Controls depressor anguli oris and mentalis. Moves the lower lip downward.

🔷Cervical branch:
Supplies the platysma. Assists in tightening skin over the neck.

Calcium Channel Blocker & Pedal Edema

একজন ৫০ বছরের hypertensive lady, বললেন — “পা ফুলে যাচ্ছে, কিন্তু BP ঠিক আছে।”
Drug history নিলাম → Amlodipine 10 mg খাচ্ছেন।

🫵Amlodipine / Diltiazem → Peripheral vasodilation → Pedal edema করতে পারে।
🛑ডোজ কমানো বা ARB যোগ করলে edema কমে যায়।

Lesson:
সব edema কিডনি বা হার্টের না— কিছু শুধু “drug side effect”।
