This issue was and is disseminated through our annual congresses, meetings, webinars and participation in many European and international venues.
The role and mission of medical doctors and their training and culture involve the care of human beings, regardless of differences in nationality, religion and wealth, and the prevention of diseases and threats to health.
This concept is also the fundamental element of the European Medical Association.
Policy makers increasingly use medical expertise for opportunistic political and economic purposes, not always consistent with goals and results favorable to health.
The role of influential doctors and medical associations has been and is increasingly important to counter denials and fake news, as in the recent experience of the covid19 pandemic and vaccines, and the increasingly serious global climate and environmental emergency.
A very important task today for medical doctors and for any medical association, national and international, is that of countering denialism in terms of nuclear conflicts and catastrophes, which can be presented and erroneously perceived as limited local events, not influential on a global planetary level.
The space dedicated to these issues in the training of health professionals is negligible, and even more neglected is in congresses, meetings and medical journals. There are many reasons, not least the fact that these ethical dissemination activities have no public or private sponsors. This issue is of paramount relevance.

The United Nations and much more the European Commission carry out important studies and project activity: but it is all too evident that the Nation States are not at all adequately responsive to rules, recommendations and even compelling diplomatic agreements.
Despite the lack of any feedback from political institutions, our European Medical Association has never stopped intervening on environmental issues, outlining the risks of nuclear wars, including local ones. We are aware that this independent information is a key intervention tool to safeguard the health and life of mankind.

behaviors by chronobiological sleep rhythm alteration. A potentially preventive strategy would be to conduct alcohol-breathalyzer tests in situ, encouraging the young people to take responsibility: the efficacy of such a measure is still far to be demonstrated. Moreover, there is growing concern to understand those interventions which, when effectively implemented, may bring reduction in the harms associated with recreational nightlife venues (Calafat et al., 2012). Such urban environments are also named movida, which originally refers to situations of animation, entertainment and youngsters' nightlife within historical centers of most European towns. This is allowing free entrance of teens in residential pedestrian precincts-car limited access areas, often used as a no-controls frank zone (Trovato et al., 2014). Their "moving" enhances loud noise from night to early morning, alike within a no-man's land, increasing the complexity of the illicit drugs phenom- enon regulation and the multiplicity of preventative perspectives (Trovato & Vezzani, 2013). We are studying 882 youngsters (523F, 359 M, range 13-30 years old), BMI 22.44 + 3.27, to investigate if the time spent at the movida has any relationship with bed timing and wake- up, hours of sleep, noise exposure, body weight and malnutrition. Alcohol intake, coffee, cigarette smoking, illicit and stimulating drugs habits, work and school
achievements are also considered. Average sleep duration (hours) is 7.26+1.17 (2-11 hours), bed timing 2.00a.m. ‡ 1.18' (0-6 hours), bed waking up 9.15 + 1.29' am (5-16 hours), with an average daily time spent within movida of 3.15° #2.29' (0-14 hours). There is a reverse correlation of BMI with the nightlife time, p>0.001, with the waking-up hour, p0.0001. These effects were remarkably greater in the 49 youngsters (5.60%) who were frankly underweight (BMI < 18.0); since these effects resemble those of night shift workers, the term mala movida (bad nightlife is
becoming of current use. According to our results, underweight and malnutrition have a straightforward association with sleep habits of very late bed timing induced by prolonged mala movida in youngsters. Moreover, underweight and malnutrition are increasingly detected in youngsters, despite the current understatements or low under- standing of the problem even by the health professionals (Hudson et al., 2013). Differently, people and health professionals have more commonly an erroneous perception of excessive weight, as also being detected in
our study. In conclusion, the challenge could be that prevention programs addressed to specific behaviors which can affect the development of underweight and teens' health should take into account the behavioral correlation of the enhancement of excessively lasting nightlife; this is facilitated by a deregulated urban environment which
induces detrimental habits and behaviors.

You will see that, whilst we are overall making good progress towards our stated 2030 targets for reducing premature deaths due to air pollution, we are not doing well neither in reducing the number of people chronically disturbed by noise pollution, nor from certain types of soil contaminations which may lead to less quality food as well. Waste and plastic in particular (including micro-plastics) remain big challenges too, next to various other forms of chemical pollution. Implications of noise and light pollution deserve a greater attention in the context of urban environments, also in the perspective of the concept of circular medicine that is our core topic since 2020.
