ANTI-FRAUD CONSTITUTION
This Anti-Fraud Constitution ("Constitution") is entered into by and Kyle Harris Kaylaum Joneshereinafter referred to as "Beneficiary," and [ODSP Office Name], hereinafter referred to as "ODSP Office," on this July 12/2023
WHEREAS, the Beneficiary has concerns regarding potential fraud and discrepancies in the handling of their personal information and benefits by the ODSP Office;
AND WHEREAS, the ODSP Office acknowledges the concerns raised by the Beneficiary and is committed to addressing and resolving these issues in a timely and effective manner;
NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, the parties agree as follows:
1. ODSP Name Discrepancies:
1.1 Issue: The ODSP Office consistently removes the Beneficiary's middle name from their records, potentially causing problems with receiving benefits and accessing services.
1.2 Possible Solutions:
1.2.1 The Beneficiary shall gather documentation with their full legal name (including middle name) from official sources (birth certificate, driver's license, etc.).
1.2.2 The Beneficiary shall document every instance where their name is incorrectly recorded by the ODSP Office.
1.2.3 The Beneficiary shall report these discrepancies to the ODSP Office in writing and request a formal review of their file to correct the name issue.
1.2.4 If the issue persists, the Beneficiary may seek legal assistance from a legal aid organization or a lawyer specializing in social security or disability law.
2. Suspicious Financial Activity:
2.1 Issue: The Beneficiary has concerns about unauthorized transactions on their accounts, potentially involving reloadable cards and a "savings account game."
2.2 Possible Actions:
2.2.1 The Beneficiary shall monitor their accounts closely for any unauthorized transactions.
