👁️ PERRLA: The Quick Nursing Guide to Pupil Assessment

As nurses, one of the most common neurological assessments we perform is checking the patient’s pupils. You’ve probably heard the term PERRLA — but what exactly does it mean? Let’s break it down simply.

✅ What Does PERRLA Stand For?
• Pupils
• Equal
• Round
• Reactive to Light and Accommodation

🧠 Why Do We Check PERRLA?

Pupil assessment gives us valuable information about the brain’s function, cranial nerves, and overall neurological status. Changes in pupil size or reactivity can signal:
• Brain injury
• Stroke
• Increased intracranial pressure (ICP)
• Drug effects (e.g. opioids, stimulants)
• Brain herniation

👁️ How to Perform the PERRLA Assessment

1️⃣ Check for Equality & Roundness
Look at both pupils. Are they the same size (2–6 mm) and perfectly round?

2️⃣ Test Reaction to Light
Shine a penlight into one eye. The pupil should constrict (direct response). The opposite pupil should also constrict (consensual response).

3️⃣ Test Accommodation
Have the patient look at a distant object, then quickly focus on something near (your finger). Pupils should constrict as they focus closer.

📝 How to Document Pupil Findings

Always document pupil size, equality, shape, and reactivity. A perfect normal assessment would read:

“Pupils equal, round, reactive to light and accommodation (PERRLA).”

If you notice any abnormalities, describe them specifically.

👉 Always assess pupils in a well-lit area, use the same penlight, and compare both eyes side-by-side.

By mastering PERRLA, you’ll be able to quickly spot early signs of neurological problems and advocate for your patient’s safety.

📌 A – Assessment:
What’s your observation?
🩺 “BP is 82/50, HR 108, SpO2 90% on 3L. He’s pale and dizzy.”

📌 R – Recommendation:
What needs to be done?
💡 “I suggest starting a fluid bolus. Would you like labs or ECG as well?”
