The case was referred to me with patient complaining pain on biting. Clinical and radiographic assessment revealed periodontal ligament widening, and vitality testing showed no response, confirming a necrotic pulpal condition. Accordingly, root canal treatment was completed as the initial phase of management.

Had the tooth been restored using a conventional restorative approach afterward, further structural compromise would have been inevitable, with a questionable long-term prognosis.

Definitive rehabilitation was therefore carried out following the 6LA🫰🏻

To manage these cases:

1️⃣ Diagnosis & Planning 🚨
• Take multiple angled radiographs
• Magnification is a must in this situation
• Consider CBCT for unclear anatomy

2️⃣ Access Cavity & Ultrasonics 🖋️
• Ensure straight-line access
• Modify access toward suspected canal using ultrasonics for safe cutting.
• Ultrasonics serve conservative dentin removal, better visualization
• Use low power and light brushing to avoid perforations

3️⃣ File Shaping 🤺
• Start with small pre-curved hand files (8,10)
• Negotiate each canal branch separately
• Establish glide path in both canals
• Use flexible NiTi files with minimal taper
• Avoid forcing instruments

4️⃣ Irrigation & Cleaning 💉
• Use NaOCl for organic tissue dissolution
• Use EDTA for smear layer removal
• Irrigate each branch separately
• Activate irrigants with ultrasonics or sonic devices
• Chemical cleaning is more important than aggressive shaping

5️⃣ Obturation 🎯
• Fit separate master cones for each canal
• Confirm radiographically
• Preferred techniques:
Continuous Wave Technique
Single cone + bioceramic sealer
• Ensure complete 3D seal and immediate coronal restoration
