‼💚💛Sweeteners and Insulin: Why “Sugar-Free” Doesn’t Always Mean Safe
‼💚💛Expert opinion from Dr. Sergey Saadi, Preventive Medicine Clinic (EnnetusMeditsiini Kliinik, ennetusmeditsiinnikliinik.ee)
In recent years, sugar has become public enemy number one for health. It’s been linked to obesity, type 2 diabetes, skin and hormonal issues, and even cancer. More and more people are avoiding sugar and choosing products labeled “sugar-free.” But does this mean these products are automatically safe?
The reality is more complex. One of the biggest metabolic problems of our time isn’t just blood sugar, but chronically high insulin levels – also known as hyperinsulinemia. Many popular sweeteners, even those with zero calories or glucose, can stimulate insulin production – and that’s where the real problem lies.
Some of the most common and controversial sugar substitutes are maltitol, maltitol syrup, and maltodextrin. These can be found in protein bars, “fitness” desserts, chewing gum, and candy. Even when the label says “0g sugar,” your body can still react. These sweeteners may cause a significant insulin response – even if blood glucose barely rises. This can promote fat storage, block weight loss, and worsen insulin resistance.
Similarly, erythritol is often promoted as the ideal sweetener: it does not raise blood sugar or insulin. However, recent research has linked high erythritol levels in the blood to an increased risk of blood clots, especially in people with metabolic disorders. Erythritol can also cause bloating and may affect the gut microbiome.
Even stevia, often seen as a natural and safe alternative, deserves a critical look. Especially the refined forms (like rebaudioside A) are widely used. Some data suggests stevia may affect sweetness perception in the brain, appetite regulation, and the composition of the gut microbiome. Refined stevia can inhibit the growth of some beneficial gut bacteria. And since raw, unprocessed stevia tastes bitter, most products use processed forms—which may carry more side effects.
So, are there any truly safe sweeteners?
Fortunately, yes. Modern research supports two sweeteners with proven safety and even metabolic benefits:
1. Allulose – A rare monosaccharide not absorbed by the body. Studies show it can lower HbA1c, reduce liver fat, and improve insulin sensitivity. Allulose does not raise blood sugar or insulin and is suitable for people with diabetes or anyone aiming to improve metabolism.
2. Monk Fruit (Luo Han Guo extract) – Hundreds of times sweeter than regular sugar, but does not affect glucose, insulin, or the microbiome. There’s no known inflammatory or digestive side effect.
What about children? Caution is needed. Even if a sweetener does not raise glucose, it may influence taste preferences and behavioral patterns. Sometimes, digestive upset, migraines, or changes in gut bacteria may occur. If used at all, choose clear, clean-labeled products and use the smallest possible amounts.
Why has the effect of sweeteners on insulin become such a hot topic?
Because metabolic disorders, obesity, and chronic inflammation are skyrocketing worldwide. Many people no longer eat sugar by the spoonful – they drink “zero” sodas, eat “sugar-free” bars, and end up with a constant insulin load that keeps the body in storage and inflammation mode.
It’s time to be honest about sweeteners: Not everything that tastes sweet is safe. And not everything “sugar-free” supports your health.
Bottom line:
When choosing a sweetener, prefer allulose and monk fruit. They do not raise glucose or insulin, do not cause inflammation, and do not harm the microbiome. Use all others – including stevia and erythritol – only consciously and in moderation.
