FDA September New Drug Approval

"A New Chapter in Urticaria Care: Meet Rhapsido (remibrutinib) — the First Oral BTK Inhibitor for Chronic Spontaneous Urticaria"

Rhapsido (remibrutinib) has been approved by the U.S. FDA as the first oral, targeted BTK inhibitor for adults with chronic spontaneous urticaria (CSU) who remain symptomatic despite standard antihistamine therapy.

This approval has the potential to reshape how patients and physicians manage a condition that has long been frustrating and unpredictable.

📍Why This Matters📍

▶️ Novel mechanism of action – Rhapsido targets Bruton's tyrosine kinase (BTK), helping curb downstream release of histamine and inflammatory mediators.

▶️ Oral, convenient dosing – 25 mg taken twice daily, with or without food.

▶️ Rapid and meaningful symptom relief

•Some patients achieved well-controlled disease as early as Week 2
• By Week 12, ~31% experienced complete resolution of hives and itch in trials vs ~10% on placebo

✔️ Be mindful of bleeding risk, especially in patients on anticoagulants — treatment may need interruption before surgical procedures.

✔️Avoid concomitant use with strong CYP3A4 inhibitors or inducers to prevent safety or efficacy issues.

✔️ Patients should avoid live or live-attenuated vaccines during treatment.

✔️ Not indicated for other types of urticaria beyond CSU in current labelling.

Fighting cancer is hard, losing muscle shouldn’t be part of it.

What is chemotherapy-induced cachexia?

A treatment-linked syndrome of unintentional weight loss, muscle wasting, fatigue, and reduced function—distinct from general cancer wasting.

Why it happens:
• Chemo → pro-inflammatory cytokines & ROS
• Activation of NF-κB / MAPK → ubiquitin-proteasome muscle breakdown
• Mitochondrial dysfunction drives atrophy, low energy
• Appetite loss, nausea, fatigue → myosteatosis & further decline

Where we are now:

• Current supports: ghrelin agonists, SARMs, omega-3s, nutrition, exercise (helpful but inadequate)
• Mitoprotective compounds look promising to protect muscle

Bottom line:

We need a combined plan—targeted pharmacology + nutrition + exercise—while research advances nanotech/smart approaches to preserve muscle and quality of life.
