🦵 Compartment Syndrome –

Compartment syndrome occurs when increased pressure within a closed fascial compartment compromises muscle, nerve, and soft tissue perfusion, leading to ischemia and potential irreversible damage.

🔹 Common Locations & Causes
• Most common: anterior compartment of the lower leg and forearm
• Usually secondary to trauma (fractures, muscle injury)
• Non-traumatic causes include:
• Thrombosis → ischemia–reperfusion injury
• Prolonged limb compression (e.g. alcohol/drug intoxication)

🔹 Clinical Presentation
• Acute onset over hours
• Pain out of proportion (P**P) – hallmark feature
• Pain with passive stretch of fingers/toes

🔹 The 6 Ps
• Pain
• Paresthesia
• Pallor
• Poikilothermia
• Paralysis
• Pulselessness (late sign)

🔹 Diagnosis
• Primarily clinical: history + serial examinations
• Compartment pressure >30 mmHg supports diagnosis
• Delta pressure = diastolic BP − compartment pressure
• Diagnosis if delta pressure ≤30 mmHg

🔹 Management
• Immediate fasciotomy to relieve pressure and restore perfusion
• Timing is critical:
• 6 hours → ↑ risk of nerve damage, muscle necrosis, amputation

🔹 Key Complication
• Volkmann contracture: ischemia → muscle necrosis → fibrosis
(classically after supracondylar humerus fractures)
