💊 When Progesterone Triggers a Dangerous Reaction

A 42-year-old woman developed DRESS syndrome—a severe allergic reaction with eosinophilia—after progesterone therapy for fertility.

Steroids didn’t fully help. But after starting a JAK inhibitor (upadacitinib), her symptoms finally improved, and steroids were stopped.

Hormones can trigger serious immune responses.

42세 여성이 난임 치료 중 프로게스테론 투여 후 DRESS 증후군(호산구 증가를 동반한 심각한 약물 반응)을 겪었습니다.

스테로이드 치료에도 불완전한 반응을 보였지만, JAK 억제제(upadacitinib) 투여 후 증상이 호전되어 스테로이드를 중단할 수 있었습니다.

Not All Asthma Is the Same – Even When the Airways Don’t React

In a study of 346 adult asthma patients across 6 hospitals in Korea, researchers found that patients less sensitive to methacholine (PC20 > 16 mg/mL) were more likely to have late-onset asthma, obesity, and frequent exacerbations—despite showing less Type 2 inflammation (like eosinophils or FeNO).

Interestingly, these patients also tended to be undertreated based on GINA guidelines.

🧠 Takeaway: Some asthma patients may not show classic airway hyperresponsiveness, but still suffer more severe outcomes. Personalized care matters.

💨메타콜린에 반응하지 않아도 위험한 천식

한국 6개 대학병원의 성인 천식 환자 346명을 분석한 결과, 메타콜린에 덜 민감한 환자들(PC20 > 16 mg/mL)은 늦게 시작된 천식, 비만, 그리고 응급실 방문 등 악화 빈도가 더 높았습니다.

놀랍게도 이들은 호산구 수치나 FeNO 등 T2 염증 지표는 낮았지만, 치료 강도는 GINA 기준에 비해 낮았고 결과적으로 더 많은 악화를 겪었습니다.

이번호에 게새된 원저 "Approach to children with IgE-mediated food allergy with a focus on oral allergy syndrome"에서는 1,660 명의 4에서 13세 사이의 소아를 대상으로 구강알레르기 증후군의 유병률과 흔한 증상, 원인 알레르겐 등을 조사하였으며 알레르겐 별 증상의 중증도를 구분할 수 있었다.
