Broken hand

28 year old male presented with pain and swelling in his left hand after an accident. Imaging shows a fracture in the third metacarpal, which is the long bone in the hand that connects the middle finger to the wrist.

The fracture is oblique, meaning the bone is broken at an angle, and it is displaced, which means the broken pieces have moved out of their normal position.

This type of injury usually happens after a strong impact, such as a fall, a direct blow, or an accident where the hand hits something hard.

These are skull X-rays of a 65 year old woman who has a history of repeated bone fractures and anemia. Because of these problems, doctors are suspicious of multiple myeloma.

To help make the diagnosis, usually perform a skeletal survey. This is a set of X-rays that looks at many bones in the body, including the skull, spine, pelvis, ribs, and long bones.
On the skull X-ray, there are many small, round dark spots. These dark areas mean parts of the bone have been destroyed. This happens because abnormal cells inside the bone are breaking it down.

The skull bones also look thinner and darker than normal. This shows that the bones are weak and fragile, which helps explain why the patient has had multiple fractures.

When the Spine Isn’t Straight: Understanding Scoliosis

The first image shows a normal lower back where the spine is straight.
The second image shows the spine bending to the side. This side bend is called scoliosis.

Scoliosis means the spine is not straight. Some people are born with it (congenital) , and others develop it later in life (degeneration). In adults, it often happens because the spine wears out with age.

Common physical signs of scoliosis

â—ŹOne shoulder or hip looks higher than the other.
â—ŹOne shoulder blade sticks out more.
â—ŹA small hump on one side of the back when bending forward.
â—ŹIn older adults, it can cause lower back pain or pain and numbness going down the leg, which is called sciatica.

Vulnerable Hips

Hip fractures are one of the most frequent findings on X-rays of injured older adults, with a significantly higher prevalence in women.

â—ŹWhy Hip Fractures are Prevalent in Older Adults.
The primary reason hip fractures are so common in the elderly due to increased fall risk and decreased bone strength.

Osteoporosis: with bones naturally lose minerals and density. A condition where bones become porous and brittle.

Physical Factors: like reduced muscle mass, poor balance, vision impairment significantly increase the likelihood of falling.

â—ŹWhy Women are More Affected

Statistics show that roughly 75% to 80% of all hip fractures occur in women.

Estrogen( a hormone) plays a critical role in maintaining bone density. After menopause, estrogen levels drop sharply, leading to rapid bone loss.

Women generally have smaller and thinner bones.

The little guy's little toe got caught in a bicycle wheel.

Findings: Displaced and angulated Salter-Harris Type II fracture of the 5th toe's proximal phalanx.

The fracture is in the first bone of the little toe, close to where the toe joins the foot.

The break goes through the growth plate (dark, empty gaps), which is the cartilage area near the end of the bone that helps the toe grow. The fracture also goes into a nearby part of the bone next to the growth plate. This called Salter-Harris Type II fracture.

Displaced means the broken parts have moved out of place. Angulated means the bone is bent at an angle instead of being straight.

Neck AP and Lateral X-ray showing Right side deviation and narrowing of the trachea due to thyroid goiter.

Goiter: An enlarged thyroid gland.

Tracheal Deviation: This is the most common sign. A large thyroid goiter typically pushes the trachea to the opposite side, which is clearly visible as a shift in the dark, air-filled column of the airway on the AP X-ray.

Tracheal Narrowing: The goiter may compress the trachea, narrowing the airway. Clearly visible on the lateral X-ray.

Broken Lower Leg.

A 14 year old male with a history of a fall. Findings include a mid shaft greenstick fracture of the tibia (larger bone) and a mid shaft transverse fracture of the fibula ( the thinner outer bone).

Greenatick fracture: Specific type of fracture common in children where the bone bends and cracks but doesn't break completely.

Transverse fracture: a straight break across the bone.
