C/O: -Severe pain in both eyes.
-History of use of herbal extract in both eyes for 4 days due to eye ache before presentation at the clinic.
-Itching, mucopurulent discharge, chemosis, and hyperemia involving the bulbar, tarsal, and palpebral conjunctiva.
-No corneal ulceration or scarring observed.

VA:
OD: 6/24
OS: 6/18

IOP:
Not obtainable due to severe ocular pain in both eyes.

Pupillary Response:
Sluggish reaction to light on external examination, both eyes.

Ophthalmoscopy:
OU: CDR 0.4

What's your diagnosis and Management plan?

C/C: –Px can't see with the left eye.
–Tearing, OS.
–Pain, OS.
–Bought an eye drop from the chemist (px doesn't know the name of the eyedrop).

VA: –OD: 6/6,.
–OS: HM@3M

EE: OD: – Pupil; reactive.
– Conjunctiva; clear

OS: – Pupil; sluggish.
–Conjunctiva; mild injection (temporally)

OPHTHALMOSCOPY:
OD; lens: transparent.
media: transparent.
C/D: 0.4

OS; lens: opaque.
media: opaque/poor red reflex.
C/D: ??

Tonometry:
OD: 14mmHg,
OS: 29mmHg

What's your Diagnosis?

Was hit with a stick by his classmate in school.

LE VA : LP

LE IOP : 10.8mmhg

C/c : Severe pain , redness, pulling sensation, cloudy vision.

Ophthalmoscopy View : No lens , pupil fully dilated.

📌 A follow-up from our previous post:

The patient returned to the clinic after 3 days.

Case history: Mild tearing.

Visual Acuity (VA):
RE: 6/24 (the affected eye)
LE: 6/5

(IOP):
RE: 12.5 mmHg
LE: 12.2mmHg

The patient reports significant improvement in comfort. Redness has reduced markedly compared to the initial presentation. He is now able to sleep well, unlike before when he could not sleep for two days.

On external examination, the cornea shows clear signs of significant healing compared to day one.

The patient was advised to continue the prescribed medications and to return for review in 5/7 days.

What do you think in this situation?

Pupil responds : very sluggish

IOP : 2.0mmhg

C/C : severe pain , tearing , photophobia, Headache and pulling sensation

Fundus : Very Hazy

PX.Case History:

The patient was hit in the eye RE, by a palm kernel.

He initially thought it was a minor condition, but it progressively worsened, with intense pain, teariness and redness. Px couldn't sleep for two days.

He also mentioned that an elderly woman advised him to instill plant leaves into the eye.

IOP.
RE: 12.2mmHg
LE: 12.5mmHg

RE: 6/36-1
LE: 6/5
