🔷PHYSICAL THERAPY:
PT is a way to get back strength and movement after an injury. It includes things like exercise, ice, heat, massage, and equipment to help return shoulder back to its normal range of motion.
👉 GOALS to be achieved with physical therapy:
◽Reducing pain and muscle tension in the scapular and neck area.
◽Improving the wrong humeral head position
◽Strengthen the muscles
◽Regain proprioception and movement automatism
◽Both NON-OPERATIVELY REHABILITATION and POSTOPERATIVE REHABILITATION of the rotator cuff involves the following principles.
➡️ Reduction of overload and total arm rehabilitation
◽It is advised to quickly use the elbow, forearm, and wrist in order to strengthen them, especially during long immobilization.
◽Scapular protraction and retraction resistance exercise.
◽Restoration of normal joint arthrokinematics
◽Posterior capsular mobilization and stretching techniques are often indicated and applied to improve internal rotation ROM.
◽Promotion of muscular strength balance and local muscular endurance

🔷MEDICATIONS: like acetaminophen and anti-inflammatory drugs
🔷 REST: to allow rotator cuff to heal
🔷 STEROID INJECTIONS: into shoulder joint, which can provide temporary pain relief
🔷 SURGERY: In some cases, they might need to take out small pieces of tendon or bone that are stuck in shoulder joint, or remove small areas of bone or tissue to give tendon more room to move.

KNEE CONDITIONS:

👉KNEE EFFUSION, sometimes called water on the knee, occurs when excess fluid accumulates in or around the knee joint.

🔘CAUSES:
◽Arthritis.
◽Injury to the ligaments or meniscus.

🔘SYMPTOMS:
Excess fluid will cause puffiness around the bony parts of the knee. It may be difficult to bend or straighten the knee.

🔷TYPICAL SYMPTOMS include:
◽Swelling
◽Stiffness
◽Reduced range of motion

🔘The following SYMPTOMS need URGENT MEDICAL ATTENTION:
◽Inability to put weight on the joint
◽ No pulse in the feet
◽loss of feeling below the knee
◽Fever

Other symptoms will depend on the UNDERLYING CAUSE. For example:
🫴 OSTEOARTHRITIS: Pain and stiffness tend to be worse first thing in the morning but often improve within 30 minutes of getting up.
🫴 RHEUMATOID ARTHRITIS: Pain and swelling often affects several joints and often symmetrically. The joint may feel hot to the touch. Pain persists longer than 30 minutes after waking.
🫴 TRAUMATIC INJURY: There may be bruising and severe pain, and it may be hard to put weight on the knee.
🫴 INFECTION: The knee may be hot, and there may also be fever.

It is very common to get backache or back pain during pregnancy, especially in the early stages.

During pregnancy, the ligaments in your body naturally become softer and stretch to prepare you for labour. This can put a strain on the joints of your lower back and pelvis, which can cause back pain.

★ AVOIDING AND EASING BACK PAIN IN PREGNANCY

Try these tips:👇

★ Bend your knees and keep your back straight when you lift or pick something up from the floor.

★ Avoid lifting heavy objects
move your feet when you turn to avoid twisting your spine.

★ Wear flat shoes to evenly distribute your weight.

★ Try to balance the weight between 2 bags when carrying shopping.

★ Keep your back straight and well supported when sitting – look for maternity support pillows.

★ Get enough rest, particularly later in pregnancy.

★ Have a massage or a warm bath.

★ Use a mattress that supports you properly – you can put a piece of hardboard under a soft mattress to make it firmer, if necessary.

★ Go to a group or individual back care class.

★ You can take paracetamol to ease back pain while you are pregnant, unless your GP or midwife says not to.

★ Always follow the instructions on the packet.

If your backache is very painful, talk to your GP or midwife. They may be able to refer you to an obstetric physiotherapist at your hospital, who can give you advice and may suggest some helpful exercises.

★ EXERCISES TO EASE BACK PAIN IN PREGNANCY

This gentle exercise helps to strengthen stomach (abdominal) muscles, which can ease back pain in pregnancy:

★ Start on all fours with knees under hips, hands under shoulders, fingers facing forwards and stomach muscles lifted to keep your back straight. For the first picture

★ Pull in your stomach muscles and raise your back up towards the ceiling, letting your head and bum relax downwards gently – do not let your elbows lock and only move your back as far as you comfortably can. For the 2nd picture

★ Hold for a few seconds then slowly return to the box position – take care not to hollow your back, it should always return to a straight, neutral position. For the 3rd picture

★ Do this slowly and rhythmically 10 times, making your muscles work hard and moving your back carefully.

★ Doing prenatal yoga or aquanatal classes (gentle exercise classes in water) with a qualified instructor can also help build your muscles to better support your back. Ask at your local leisure centre.

How Environmental Factors Can Impact Personal Health.

Environmental health advocacy is a top priority in the public health field, as environmental factors continue to adversely affect individual and population health.

For example, consider the looming threat of climate change. Environmental health researchers say that climate change may result in changed disease patterns, making communities susceptible to different illnesses. Additionally, climate change has been linked to an uptick in floods, wildfires, and superstorms, all of which can play profound roles in harming human health.

Poor industrial hygiene is another example of how environmental factors can impact wellness. As Verywell Health notes, “Many of the materials needed to advance industries and technology, like heavy metals or even some plastics, can also hurt the human body and even lead to serious medical conditions.” In other words, industries that are irresponsible with manufacturing and waste disposal may be poisoning the natural world, impacting our physical well-being.

Environmental health isn’t just a matter of individual wellness; it’s also a matter of community well-being. Simply put, the conditions in and around our homes, schools, playgrounds, and workplaces can have a major impact on our families and neighbors.

For example, factors such as pollution and litter can make residents less inclined to go outside for exercise and recreation, potentially making the entire community more prone to disease.

Aspects of the built environment, such as infrastructure, can also impact community health. For instance, areas with poorly maintained roads have higher risks of car accidents that result in injuries, and inadequate sanitation infrastructure can increase the prevalence of disease in local populations.

Environmental Factors That Can Impact Your Health

A number of specific environmental issues can impede human health and wellness. These issues include chemical pollution, air pollution, climate change, disease-causing microbes, lack of access to health care, poor infrastructure, and poor water quality.

1. Chemicals
Different chemicals can impact human health in different ways, and often, exposure to dangerous or foreign substances creates health vulnerabilities. The chemical safety field is concerned with minimizing the effects of both natural and synthetic chemicals. More specifically, chemical safety seeks to safeguard human and environmental health from exposure to potential toxins; it focuses on chemicals that exist in the ambient world, as well as synthetic chemicals that are used in industrial processes or are the by-products of manufacturing activity.

Public health officials who work in chemical safety often focus on toxicology, which is the study of substances that have a toxic effect on the human body, whether when ingested or when absorbed through natural surroundings.
Some examples of dangerous chemicals in the environment are heavy metals and toxins that find their way into the water supply and harmful pesticides that make their way into the food supply chain.

2. Air Pollution
Studies have shown that air pollution effects on humans are a significant public health concern, not only because of their role in climate change, but also because exposure to air pollution can increase morbidity and mortality.

When environmental scientists talk about air pollution, they’re describing any solid or liquid particles, as well as gases, that are suspended in the air and have harmful or poisonous effects. When we think about air pollution, we tend to think in terms of human-caused pollutants, and rightly so: Some of the most dangerous and ubiquitous examples of air pollution include car and truck exhaust, as well as polluting by-products of industrial processes. However, many natural air pollutants can cause health concerns; for example, pollen and mold spores are often linked with asthma and allergies.

3. Climate Change and Natural Disasters
Another environmental issue with serious ramifications for human health is climate change, along with the increase in natural disasters that has accompanied the shift in Earth’s climate. The National Environmental Health Association lists climate change as the single biggest human health threat of the 21st century.

Climate change disrupts the natural world in a number of ways that can impede health and increase vulnerability to disease; these include increases in the planet’s temperature and more frequent heavy rains and runoff. The various impacts may result in greater vulnerability to nervous and respiratory diseases, diarrhea, and more.

Additionally, climate change increases the frequency of natural disasters, often having a ravenous effect on homes and communities and sometimes resulting in loss of life. Wildfires, hurricanes, cyclones, and droughts are just a few examples.

4. Diseases Caused by Microbes
Diseases caused by microbes — also known as microorganisms — present another area of public health concern. Trillions of microbes exist within the human body, and they also live in water, soil, and air. Most of them have no negative health effects, and many microbes perform important biological functions, such as supporting digestive and immune health, according to the National Human Genome Research Institute. Bacteria, viruses, and fungi are types of microbes.

Harmful microbes, which are more commonly called pathogens or germs, can infect humans and cause illness. Eating is one of the primary ways in which humans can contract diseases caused by microbes. One example is food poisoning from E. coli, a type of bacteria that can be found in the environment and in foods, and which can cause respiratory illness, urinary tract infections, and other adverse health effects.

There are also a number of harmful microbes that live in the soil. Humans can come into contact with them by ingesting them (through contaminated food) or through inhaling them (through soil particles in the air). Tetanus and botulism are examples of diseases caused by soil-borne microbes.

5. Lack of Access to Health Care
Another environmental factor that contributes to disease and overall poor health outcomes is living in an area with a lack of access to health care services.

According to Healthy People, “Many people face barriers that prevent or limit access to needed health care services, which may increase the risk of poor health outcomes and health disparities.” Some factors that contribute to lack of access to health care are economic instability, unreliable transportation to clinical facilities, and simple lack of education about the importance of preventive care.

Individuals who don’t have access to health care services (including pharmaceuticals as well as dental care) are at a higher risk for chronic conditions, which may include asthma, cancer, diabetes, or heart disease.

6. Infrastructure Issues
Infrastructure issues can also have a major impact on community health. Examples include the following:

*Poorly maintained roads (increasing risk of car accidents)
*Lack of access to clean drinking water
*Lack of local health care infrastructure, such as clinics capable of giving vaccinations.
Overcoming poor infrastructure requires the right allocation of resources from local, state, and federal governments, as well as a qualified workforce to keep infrastructure properly maintained.

7. Poor Water Quality
Verywell Health reports that across the world, more than 780 million people don’t have access to clean drinking water; shockingly, about a third of the world’s population lacks access to proper sanitation services (e.g., clean bathrooms). The health implications are sobering: Each day, more than 2,200 children die due to diseases caused by poor water quality.

A number of factors can contribute to poor water quality, including industrial waste and pollution, lack of access to proper water treatment and sanitation services, and outdated plumbing infrastructure.

8. Global Environmental Issues
Global environmental issues account for more than 12.6 million deaths each year, according to WHO data. Along with the issues mentioned here, these can include soil pollution, ultraviolet radiation, and biodiversity loss. According to the data, more than 100 illnesses and injuries can be directly linked to environmental health concerns. Often, these issues have the greatest impact on communities that are poor and already have significant health care vulnerabilities.

There’s a collection of nerve roots at the bottom of your spinal cord that affect your legs and bladder. When an injury or herniated disk or other problem compresses those nerves, then that causes pain, weakness and incontinence.
Cauda equina syndrome is a medical emergency. You should see a healthcare provider immediately if you have symptoms.

— OVERVIEW
What is cauda equina syndrome?
Cauda equina syndrome is the compression of a collection of nerve roots called the cauda equina. Nerves send and receive electrical signals all across your body. The collection of nerve roots, shaped like a horse’s tail, is located at the bottom of your spinal cord.
Because of the cauda equina nerves, you can move and feel sensations in your legs and urinary bladder.
Compressed cauda equina nerves can cause pain, weakness, incontinence and other symptoms. This syndrome can cause permanent damage, including paralysis, if left untreated. Quick treatment might prevent permanent damage.

— Is caudequina syndrome life-threatening?
Cauda equina syndrome is not life-threatening. But it can permanently damage your body, affecting your health and quality of life. Symptoms may include leaking p*e or p**p and problems having s*x. Surgery might treat several of the symptoms if performed immediately.

— There are two types and two classifications of cauda equina syndrome.
★ The syndrome is acute or chronic, and it’s either complete or incomplete.

— Types of cauda equina syndrome
Acute cauda equina syndrome

★ Severe symptoms start suddenly. You’ll likely need surgery within 24 to 48 hours.

★ Chronic cauda equina syndrome;
This name means long-lasting cauda equina syndrome. It describes two scenarios:

— If you’ve had symptoms of cauda equina syndrome for a period of time before you see a healthcare provider, it’s labeled chronic.
If the surgery doesn’t fix your nerves and there is permanent damage, you’ll likely have symptoms for the remainder of your life. Your healthcare provider calls these symptoms chronic.
Emergency surgery might stop permanent damage. See your healthcare provider if you have symptoms.

— Classifications of cauda equina syndrome
Complete cauda equina syndrome
Complete cauda equina syndrome causes urinary and/or bowel retention or incontinence. Retention means that you’re unable to p*e or p**p, and incontinence means that you can’t stop yourself from p*eing or p**ping. It affects about 60% of people with cauda equina syndrome.

— Incomplete cauda equina syndrome
This affects the other 40% of people with cauda equina syndrome.
Typical symptoms includes; loss of urgency or increased urgency sensation in the bladder and bowels without retention or incontinence. This means you can’t feel that you have to p**p or p*e, or you feel the sensation stronger than before.

— How rare is cauda equina syndrome?
Experts estimate the cauda equina syndrome affects around 1 in 65,000 people. It affects males as equally as females.

— SYMPTOMS AND CAUSES
How do I get cauda equina syndrome?
There are a number of reasons why the nerve roots that make up the cauda equina could get compressed, including:
★Birth anomalies.
★Lumbar spinal stenosis.
★Injuries to the lower back like from a car accident or gunshot wound.
★Herniated disk in the lumbar region. This is the most common cause.
★Postoperative lumbar spine surgery complications.
★Spinal arteriovenous malformations (AVMs).
★Spinal hemorrhages.
★Spinal infections (such as meningitis) or inflammation.
★Spinal lesions or tumors.

— What are the symptoms of cauda equina syndrome?
There are several red flags that you might have cauda equina syndrome:
★Numbness or different sensations in the backs of your legs, butt, hip and inner thighs (your saddle area, as in the parts of your body that would touch the saddle if you were on a saddled horse).
★Pain in your back and/or legs (sciatica).
★Problems having s*x.
★Urinary and/or f***l incontinence.
★Urinary retention.
★Weakness or paralysis in your lower extremities.

— You might experience more symptoms in addition to the red flags.
They include:
★Lower back pain.
★Burning, prickling, tingling or numbness in the lower limbs (paresthesia).
★Reflexes issues.
See your healthcare provider if you have these symptoms. You might be tested for cauda equina syndrome and sent to a neurosurgeon or orthopedic spine surgeon. Surgery may help you by stopping additional damage to your nerves.

— DIAGNOSIS AND TESTS
How is cauda equina syndrome diagnosed?
Your healthcare provider will diagnose your cauda equina syndrome by interviewing you about your symptoms, assessing your physical abilities, performing tests and ordering imaging tests.
They’ll assess your abilities by having you:
★Stand.
★Sit.
★Walk on your heels.
★Walk on your toes.
★Bend forward.
★Bend backward.
★Bend to the sides.
★Lie down and lift your legs.

— This information helps your healthcare provider assess your:
★Stability.
★Sensation.
★Strength.
★Reflexes.
★Alignment.
★Motion.
★Your healthcare provider might then perform a re**al exam to check your a**l muscles.

— , your healthcare provider might order the following tests:
★FinallyComputed tomography (CT) scan.
★Magnetic resonance imaging (MRI).
★Myelogram.

— Using these imaging tests should allow your provider to see your:
★Bones.
★Nerve roots.
★Spinal cord.
★Displaced spinal cord or nerves because of herniated disks, bone spurs and tumors.

— You’ll be diagnosed with cauda equina syndrome if you have two sets of symptoms:
★Bowel, bladder and/or s*xual problems.
★Paresthesia of the backs of your legs, butt, hip and inner thighs.

— What questions might a healthcare provider ask to diagnose cauda equina syndrome?
Your healthcare provider will likely have many questions for you. They need to understand what’s happening to you so they can do the right tests, order the correct imaging tests and give you an accurate diagnosis. Questions might include:
★What are your symptoms?
★How long have you had these symptoms?
★What medications do you take?
★Have you ever injured your spine?
★Which symptom is worse?
★Have you ever had a herniated disk?

— MANAGEMENT AND TREATMENT
How do you fix cauda equina syndrome?
New (acute) cauda equina syndrome is an emergency. If your healthcare provider determines that you have this condition, you’ll likely go into surgery within 24 hours. Surgery will move whatever is compressing your nerve roots so they can heal. The type of surgery is called a lumbar laminectomy.

— Some medications can improve your bowel and bladder control. They include:
★Hyoscyamine (Levsin®).
★Oxybutynin (Ditropan®).
★Tolterodine (Detrol®).

—★★★You may need to see a physical therapist and occupational therapist. Your physical therapist can help you regain your strength after surgery, and an occupational therapist can help you with everyday activities.

— PREVENTION
How can I prevent cauda equina syndrome?
It may not be possible to prevent all of the causes of cauda equina syndrome. But, you might be able to reduce your risk of a herniated disk, the most common cause of the syndrome.

— Try the following tips:
★Avoid wearing high-heeled shoes. These types of shoes can misalign your spine.
★Stop smoking.
★Avoid using other to***co products as well because they can weaken your disks.
★Exercise. Try exercises that strengthen your back and abdominal muscles.
★Lift properly. Bend your knees and keep your back straight when you lift something instead of bending at your waist.
★Maintain a healthy weight. Extra weight adds extra pressure to your lower back.
★Practice good posture. That will reduce strain on your spine.
★Stretch. Stretch your body periodically, especially after you’ve been sitting for a long time.

— OUTLOOK / PROGNOSIS
What are the long-term effects of cauda equina syndrome?
Go to the emergency department if you have symptoms of cauda equina syndrome. You might need surgery right away. Your surgeon will move whatever is pressing down on your nerve roots so they can heal. But surgery doesn’t guarantee that you won’t have any long-term issues. Your nerves might not heal correctly or heal at all. You might have permanent damage. The best way to prevent that is to report to the emergency department and get surgery as soon as possible. The earlier the treatment, the better.

The faster you have surgery, the less time it will take your nerves to heal. If the surgery does help your nerves, it might take you a while to feel like yourself again. As the nerves heal over months or even years, you’ll slowly regain control over your bowels and bladder, the pain might lessen and the weakness might go away.

— LIVING WITH
How do I take care of myself?
The best thing you can do for yourself if you have symptoms is to see a healthcare provider. Continue getting help from healthcare providers after you have surgery, including:
★Physical therapists.
★Occupational therapists.
★Sex therapists.
★Primary care healthcare providers.
★Psychiatrists.
★See if your family and friends are willing to help you as well. You’ll need support.

— What questions should I ask my healthcare provider about cauda equina syndrome?
★What type of cauda equina syndrome do I have?
★What classification of cauda equina syndrome do I have?
★How quickly do I need surgery?
★How long will the surgery take?
★How long will I be in the hospital after surgery?
★Do I have a herniated disc.

N/B; Cauda equina syndrome affects about 1 out of 65,000 people. It’s uncommon, but it’s important to pay attention to the red flag symptoms because they might indicate various health issues. Be on the lookout for numbness in your saddle area, pain in your back and legs, problems with p*eing or p**ping, difficulties with s*x and weakness in your extremities.
