📌 PARA SA NEW REGISTRATION at REACTIVATION
Magdala ng photocopy ng kahit ISA (1) sa mga sumusunod na valid ID:
1. PhilSys National ID
2. Postal ID
3. PWD ID
4. Student ID o Library Card (may pirma ng school authority)
5. Senior Citizen ID
6. LTO Driver’s License / Student Permit
7. NBI Clearance
8. Philippine Passport
9. SSS / GSIS ID o UMID Card
10. Integrated Bar of the Philippines (IBP) ID
11. Professional Regulation Commission (PRC) License
12. Certificate of Confirmation mula sa National Commission on Indigenous Peoples (NCIP)
13. Iba pang valid government-issued ID
📌 PARA SA CHANGE OF NAME / STATUS
• Marriage Contract
• Court Order na may Certificate of Finality
📌 PARA SA CORRECTION OF ENTRY
• Birth Certificate
⚠️ MGA PAALALA:
• ❌ Hindi tinatanggap bilang valid ID ang:
Company ID, Cedula, Barangay Clearance, at PNP Clearance
• ✅ May prayoridad ang Senior Citizens, Persons with Disabilities (PWD), at mga Buntis
• 🧒 Ang mga mag-eedad ng 15 taong gulang sa o bago ang November 1, 2025 ay maaari nang magparehistro
