June 5,2026 || LGU-Tabuk -CHSO staff headed by Dr. Henrietta Bagayao conducted medical and dental consultation with laboratory and pharmacy services in line with PHIC programs on Yaman ng Kalusugan Program(YAKAP) and Guaranteed Accessible Medicines for Outpatient Treatment(GAMOT) following the directives of the City Mayor Darwin C. Estrañero to bring health services closer to the people.

Casigayan in line with the Yaman ng Kalusugan Program(YAKAP) of the Philhealth and PuroKalusugan of the DOH. CHSO MAIN PCF caters to Brgys Dagupan Centro, Dagupan Weste, Casigayan and Magsaysay.

CHSO-Main Primary Care Facility Operation Tuli 2026

For the whole month of April and May, every wednesday is set as tuli day. Residents of Brgys Dagupan Centro, Magsaysay, Dagupan Weste and Casigayan are advised to visit CHSO Main for appointment schedule. Residents of other brgys are encouraged to visit their own health centers for their circumcision schedules.

[November 25-26, 2025 ].The CHSO Olympics 2025 unfolded as a vibrant celebration of health, unity, and Filipino culture. The two-day activity showcased not only athletic skill but also the camaraderie of Tabuk’s health workers. The vibe was electrifying as they took part in traditional Pinoy games such as Patintero and Shatong, and modern favourites like cheerdance and relay race. These activities highlighted both physical fitness and cultural pride, reminding everyone that wellness is holistic—rooted in body, mind, and community spirit.

𝑻𝒂𝒃𝒖𝒌 𝑪𝒊𝒕𝒚 𝑯𝒆𝒂𝒍𝒕𝒉 𝑺𝒆𝒓𝒗𝒊𝒄𝒆 𝑶𝒇𝒇𝒊𝒄𝒆 𝒃𝒂𝒈𝒔 𝒎𝒖𝒍𝒕𝒊𝒑𝒍𝒆 𝒂𝒘𝒂𝒓𝒅𝒔 during the 2025 GAWAD KALUSUGAN of the Department of Health - CHD CAR held last October 29 2025 at Santorini Ballroom, Hotel Elizabeth Gibraltar Rd., Baguio City.
The Gawad Kalusugan Awards is an annual gathering to honor LGUs excellent implementation of different health programs.

City Health Officer Dr. Henrietta S. Bagayao humbly and gratefully accepted the DOH Gawad Kalusugan award in Baguio City. She said that these recognitions are not merely a testament to our excellent programs and policies—it is also a tribute to the unwavering dedication of our health workers, barangay partners, and community leaders who continue to champion health in every corner of the city. She also challenged all health workers in the city to sustain the gains we have achieved and to work with compassion and dedication for the realization of our vision of becoming the "Center of Excellence for Health in the region by 2030.
