In celebration of ๐ก๐๐ง๐๐ข๐ก๐๐ ๐๐๐ฆ๐๐๐๐๐๐ง๐ฌ ๐ฅ๐๐๐๐ง๐ฆ ๐ช๐๐๐ and its commitment to support inclusive education, the Concepcion Scholarship Foundation, Inc. (CONSFI) is pleased to open the ๐๐ฝ๐ฝ๐น๐ถ๐ฐ๐ฎ๐๐ถ๐ผ๐ป ๐ณ๐ผ๐ฟ ๐๐ต๐ฒ ๐๐ผ๐๐ฟ๐๐ต ๐๐ฎ๐๐ฐ๐ต ๐ผ๐ณ ๐ฆ๐ฐ๐ต๐ผ๐น๐ฎ๐ฟ๐, exclusively for Persons with Disabilities (PWDs) college students in a two-year or four-year degree program for SY 2026-2027.
CONSFI recognizes the contributions, strength, and potential of persons with disabilities. Through this scholarship program, the Foundation aims to provide educational support to deserving PWD students so they can continue their studies, reach their goals, and help their community.
QUALIFICATIONS:
1. Must be a permanent resident of Concepcion, Tarlac.
2. Must be a Person with Disability (PWD) belonging to any of the following categories:
โข Visual Disability โ blind, cross-eyed, or with severe and uncorrectable visual impairment
โข Orthopedic Disability โ amputated limb/s, polio, dwarfism, or other orthopedic impairments such as cerebral palsy and muscular dystrophy
โข Hearing Disability โ deaf or deaf-mute
โข Speech and Language Disability โ mute or with speech and language impairment
3. Must be enrolled as a College student in a two-year or four-year degree program for SY 2026-2027.
DOCUMENTARY REQUIREMENTS:
1. CONSFI Application Form and Data Consent Form

OR you may get the forms personally at the Municipal Legal Office. Kindly fill out the forms properly and accurately.
2. Photocopy of PSA Certificate of Live Birth / Certified True Copy of Certificate of Birth from the Local Civil Registrar, and additional requirement if married: Photocopy of PSA Certificate of Marriage / Certified True Copy of Certificate of Marriage from the Local Civil Registrar
3. Barangay Certificate of Residency and with No Derogatory Record
4. Photocopy of a valid PWD ID (front and back) with 3 specimen signatures.
Additional: Photocopy of another valid Government-issued ID (front and back) with 3 specimen signatures. ID must indicate name, residence address, and bear a signature.
5. Certified True Copy of Certificate of Enrollment/Registration (1st Semester, SY 2026-2027) indicating the semester or school year, course, subjects with corresponding units, and a breakdown of fees.
6. a) Income Tax Return / Certificate of Employment with Income (if the applicant is working)
b) Income Tax Return / Barangay Certificate of Low Income or No Income of the parent/s or guardian supporting the applicantโs study (if the applicant is not working)
Submit the complete requirements to Ms. Patrisha Quiambao at the Municipal Legal Office, 2nd Floor, Concepcion Municipal Government Center, from Monday to Friday, 8 AM to 5 PM, personally or through a representative (with Authorization Letter and Photocopy of Representative's Valid ID)
PLEASE BE ADVISED THAT WE WILL NOT ACCEPT INCOMPLETE REQUIREMENTS NOR ALLOW PARTIAL SUBMISSION OF THE REQUIREMENTS.
