- Certificate of Live Birth
- For widow/er (Biyudo/Biyuda)
Certificate of Death of the former spouse
- Valid ID
- Cedula
- PSA birth Certificate sa mga anak (if sa mabini natawo)

PHILSYS NATIONAL ID REGISTRATION & PRINTING OF ePHIL ID IN MABINI, DAVAO DE ORO

National ID registration will be done in the following schedules: 👇
JANUARY 22, 2025 (Mabini Municipal Hall Atrium)
JANUARY 23-24, 2025 (Cor Jesu Institute Technology Mabini, Davao de Oro)

The Registration will Cover :

✅CHILDREN 4 YEARS OLD AND BELOW:
- Birth Certificate of the Child
- National I.D/e-phil/ digital ID of parents or guardian.

✅ INDIVIDUALS 5 YEARS OLD AND ABOVE:
- Bring Birth Certificate PSA/Local Copy
& Marriage Certificate (if married).

✅ DAMAGED PHIL ID CARDS:
- National I'd
- Damaged PHIL ID must be surrendered to PSA personnel at the time of registration.

✅For E-PHIL ID PRINTING
- Bring your National ID transaction slip.
