Please have CLEAR SCANNED COPIES (in JPG, JPEG, or PNG format) of the following:
1. PSA Birth Certificate
2. Passport-sized ID picture or 2x2 picture with name tag on a white background.
3. Filled out copy of the PNPA BMI Form. Visit any private clinic or a government health center to measure your height and weight and calculate your BMI. Theyโll also fill out the rest of the medical information on the form for you. (No other medical test needed).
