Rolling Veins: Fact or Fiction?

Whenever I have a class of students new to the Medical Profession I hear a lot of different stories about all of the bad experiences they have ever had in their lives. One common and very interesting conversation that seems to always take place in the first class is always about how people experience a Phlebotomist sticking them over and over because they have rolling veins. When I am asked how I deal with rolling veins because I am a full-figured woman, I literally show confusion all over my face. Now I have been a Professional Phlebotomist for more than 20 years. I have stuck Patients from all walks of life. I have stuck Patients from Neo Nate’s to Morbidly Obese patients. I have worked with Chemo Therapy Patients, AIDS Patients, L&D Patients, former and current drug addicts, ICU and PICU patients.

I have NEVER seen or felt a vein roll. I know that there are going to be some people that disagree with me and may even want to say that I have not stuck the patients that you have and I know nothing. They may also ask how would I know that when they stuck someone that the veins did not actually roll? Okay I will accept that fact that I have not stuck millions of people to compile the data I have but what I do have is an expert opinion of the proper way a Professional Phlebotomist is supposed to stick.

Take a look at the photo. This is the proper way to anchor the vein and insert your needle. Now if you are anchoring with your thumb on top and have a good grasp with your four fingers under the site you are sticking I GUARANTEE your vein will not do that rolling thing. You may want to ask how I can GUARANTEE that the vein won’t do that rolling thing in every patient.

Well here is my response to that: If you don’t know how to anchor a vein that you just palpated then maybe a few practice days are in order! If you were not taught how to anchor a vein then my suggestion to you is to go to a class and refresh your skills or buy a new class with a better Instructor. I dare you to try this anchor and then tell me it did not stop that phantom Rolling Vein! Now I know we all stick differently and we all have different techniques but Anchoring should be UNIVERSAL.

I know that it is possible for all Phlebotomist to miss a vein because we are not perfect! I also understand that we may want to come up with different yet colorful reasons why we miss the vein completely. I have heard so many different reasons why a Phlebotomist missed my veins which is every time I go to the lab for blood draw.

One Phlebotomist said, ‘Your veins are running all over the place’. Another Phlebotomist said, ‘Your veins are having a seizure because you have a lot of fatty tissue and they can’t breathe’. The funniest one yet said, ‘I think your veins are not fond of the bright light so I am going to have to turn the lights down and take your blood that way because you know our body reacts to light differently.’ Yes weird, funny, very questionable but all true!

The next time a Phlebotomist says to you and before you say it to another Patient, ask these question to yourself:

A. If veins roll when they are about to be stuck, once they roll away will they ever roll back?
B. At any point can a rolling vein get tangled around the other Veins, Tendons, or Muscles in the arm?

Do you honestly think that veins roll because they are scared of you or after you palpate you don’t hold the skin taut and hold your anchor properly?

I dare all of you to try this and tell me, no SHOW me that it does not work!
