A large study of 3,035 implant-based reconstructions after nipple-sparing mastectomy shows low complication rates overall. Smoking, radiation therapy, and incision choice were the top predictors of risk. Direct-to-implant reconstruction and ADM use were safe options. These findings support safer, more personalized approaches to breast reconstruction decision-making.
