Things that stood out to me after hearing the cross examination with psych NP Rebecca Jollotta says with the Lindsay Clancy trial:
-getting progressively worse over months—>insomnia worsening after SSRI initiation (up for 48hr straight)..reported not feeling tired despite this
-turned away from Women & Infants Hospital/postpartum program b/c they thought her symptoms were due to polypharmacy
-NP brings up possibility of underlying bipolar disorder (I believe she was the only provider that brought this up?)
-Lindsay reported in the end of Nov being disoriented, forgetful, restless, panicked, intrusive thoughts, and not feeling connected to her body
-did not meet inpatient treatment d/t not having SI plan
-reporting SI

—>some more thoughts: meeting in person is better when it comes to assessing a pt, however, important to note that telemedicine expands access to care for pts (for example no need for childcare arrangements/driving to destination etc..more flexible)

Pharmacist Thoughts on Lindsay Clancy’s Meds 💊

Lindsay Clancy trial med 💊 timeline—>I made a vid about the trial and my thoughts. Some more thoughts:

-SSRIs are not used for postpartum psychosis (good choice for PPD esp when breastfeeding)…this requires inpatient treatment
-I’m assuming the Zoloft was stoped when the Prozac was added? But Prozac is the most activating SSRI and I would not rec this in pt who has severe insomnia
-Ambien and Benzos can actually cause hallucinations in some pts (rare; imo benefits>costs w/severity of her symptoms)
-I would have thought the Seroquel would have helped but again, she imo likely required inpatient treatment and possibly had polypharmacy working against her. The titration? was also incredibly slow for some reason
-lamotrigine does not work for acute mania. It is mainly used for bipolar depression and maintenence
-I would not prescribe a pt with SI a TCA due to how easy it is to die from an overdose

—>reiterating that these meds could be completely appropriate for a pt given their dx/reported symptoms
