In a major meta‑a**lysis published in Anesthesiology (July 2025), patient‑reported intraoperative pain during cesarean delivery under spinal or epidural anesthesia was surprisingly common. About 17% of patients experienced pain, and epidural top‑ups were associated with the highest rates. The findings highlight the need for improved preoperative discussion, pain monitoring, and anesthesia planning to improve both outcomes and patient experience.

📣 A new report from the Commonwealth Fund shows that maternal deaths in the U.S. are still on the rise. Over 6,000 women died from pregnancy-related causes between 2019 and 2023, with Black and Indigenous women facing the highest risks. Many of these deaths happen after childbirth and could be prevented. The report calls for better access to care, extended Medicaid coverage after birth, and stronger state review systems to save lives. AJOG Expert Review in Labor: A critical evaluation of the external and internal maneuvers for resolution of shoulder dystocia - A flowchart for the management of shoulder dystocia
(1) Both external maneuvers (McRoberts maneuver±suprapubic pressure) and internal maneuvers (rotational methods or posterior arm delivery) can be used as the primary method according to the accoucheur’s judgment and experience and availability of assistants. (2) If this fails, the accoucheur can then try the other approach. (3) If all of the above common maneuvers have failed, review the reason of failure. If the failure was caused by poor technique, a repeat attempt may be tried. If the accoucheur cannot insert his/her hand into the va**na, but the posterior axilla is within reach, (4) try the posterior axilla (±sling) traction or (5) roll the mother over into an all-fours position and reattempt hand insertion for internal maneuvers. (6) If the accoucheur can insert the hand into the va**na and the posterior shoulder is not high but the posterior forearm cannot be reached either because it is behind the chest or the elbow is not flexible, then posterior axilla (±sling) traction is the best option. (7) If the posterior shoulder is out of reach, then use a long right-angle forceps to reach it and form a sling for posterior axillary traction, otherwise use more invasive methods. https://ow.ly/k9Rv50WxRLG
