Our Office ONLY Uploads the Dr Recs as a PDF--- if we did your upload to OMMA -- please call/text me at 918-521-9349 and i will get the Dr Rec Reuploaded to OMMA.

REJECTION MESSAGE:
Physician Recommendation Form - Document Format – Incorrect File Type
The submitted document is in an unsupported format. Acceptable formats include pdf, jpeg, png and gif. File size must not exceed 3MB. Please correct and resubmit your application within 30 days. Failure to resubmit your application within 30 days will result in the cancellation of the application and the expiration of the related license. License fees are nonrefundable.
NOTE: Unless OMMA determines otherwise, an application resubmitted with errors that are not clerical will be denied per 63 O.S. § 420(H).

Sorry -- I have not posted in a while.
I have been updating our Dr Referral System since OMMA Created a New MedPortal.

If you get an AUTH ERROR when you try to log into OMMA -- Per Hippa you will need to Call OMMA and have them remove the AUTH ERROR.

This will have to be done Before you can apply for your OMMA MedCard or Employee Credentials.
