Is this against code or just incredibly dumb?

I am commissioning a large piece of equipment and our “electricians” ran power from a generator to the equipment. 3 phase with 3 conductors on each phase, but for some reason they taped each wire coming off a phase 3 different colors instead of keeping each phase the same color. Myself and our safety guy are pretty pi**ed about this for the obvious safety concerns, especially since one guy wired the generator side and a different guy wired the breaker side. The potential safety issues are obvious.

Long story short, is there a code this violates or just incredibly dumb?

I'm mainly residential, so don't haze me to bad. But why is it necessary to have 3 exits so close together

Does this splice kit install look correct?

My contractor added a splice kit in order to move a bathroom fan. He’s not a licensed electrician so wanted to see if anyone noticed anything wrong with this. I would get an electrician out to confirm it’s okay but the contractor already drywalled over it

Do I have 220 v or just 120

I moved and bought a used washer / dryer combo, I saw the plug was different ,now they tell me that is a 220 connection because the dryer requires 220, so Im not sure if I have 220 or just 110 , how screwed am I ?

Just... I don't think this is up to code.

I work maintenance for a property managment company. Was at a home inspection today and the inspector said the knockout needed to be covered. He didn't take the cover off which was not screwed on. After he left I pulled the cover to see the make of the panel so I could note it on the work order.
I have no idea what I am really looking at but this doesn't seem right. Is this set up likely to hurt someone? Thanks
