9. Proof Of Financial Means - Bank Certificate (w ADB) -Bank statement (atleast 6 mos) - ITR 10. Proof of Strong Ties in the Philippines - Business Documents , Investment Certificate, Car OR CR, Land Title for minor: Leave of Absence in School, Certificate of Enrollment, School ID

12. Old Passport with Visa ( photocopy of pages with previously used visa)
