What Each One Covers & When to Use It

Medical coding can feel overwhelming at first, especially when you're staring down a stack of coding books. Here's a quick breakdown of what each code set does and when to use it:

📘 ICD-10-CM – Diagnosis Codes
🔸 Used by all healthcare providers and facilities
🔸 Reports WHY the patient needed care (medical necessity)
🔸 Code format: A12.3K5A (up to 7 alphanumeric characters)

📘 CPT – Procedure Codes
🔸 Used by physicians and outpatient care facilities
🔸 Reports WHAT was done for the patient
🔸 Code format: 12345 (always 5 digits)
🔸 Common in ERs, outpatient surgery centers, and clinics

📘 ICD-10-PCS – Inpatient Procedure Codes
🔸 Used only by hospitals for inpatient facility reporting
🔸 Reports procedures performed by the facility, not the provider
🔸 Code format: 012B4LZ (always 7 characters)

📘 HCPCS Level II – Supplies & Service Codes
🔸 Used for services/supplies not covered in CPT (transportation, DME, injections, etc.)

💡 What is E/M Coding?
E/M codes are used to bill for patient visits, like office appointments, hospital stays, or ER visits. These codes reflect the level of care provided.

🔑 The 3 Key Parts of E/M Coding:
1. History: What details about the patient’s condition were recorded?
2. Exam: What physical examination was done?
3. Medical Decision Making (MDM): How complex were the provider’s decisions?

📚 How to Code E/M:
1. Where was the visit?
• Office, ER, hospital, or telehealth?
2. What level of service?
• Problem-focused, expanded, or comprehensive?

3. Does the documentation match?
