Dominic Howard shares his thoughts on Penumbra’s computer-assisted vacuum thrombectomy offering. The technology, he comments, provides a “multi-level, all-in-one treatment” to address the “major health concern” that is acute limb ischaemia (ALI).

Howard—in conversation with Interventional News at the Vascular Society for Great Britain and Ireland (VSGBI) 2023 annual scientific meeting (22–24 November, Dublin, Ireland)—notes that there are data to suggest an endovascular-first approach is potentially safer than open intervention in ALI patients, with a slightly lower mortality over five years. While an open technique has some benefits, Howard stresses that this is a blind procedure that often results in incomplete clearance of thrombi. Reintervention and amputation rates are high and vessel trauma is common, he adds.

Howard details how computer-assisted vacuum thrombectomy has changed his algorithm for the endovascular management of ALI, offering the possibility of complete clot clearance in one setting with no requirement for thrombolysis. He also comments on the versatility of computer-assisted vacuum thrombectomy devices in the below-the-knee interventions, which he notes are “far more common than we might think”.
The vascular surgeon remarks that Penumbra’s Lightning 7 in particular has offered powerful aspiration over the last few years. He also considers what is next for the technology, highlighting the benefits of a new device—currently available in the USA—that “allows more rapid clearance of clot”.

In a recent interview, Marie Josee van Rijn from Rotterdam, the Netherlands and Emma Wilton from Oxford, UK, provided insight into the transformative power of interventional approaches in the treatment of DVT. Both van Rijn and Wilton agree that mechanical thrombectomy has enabled their centers to achieve an optimal patient pathway by providing interventionalists with a wider treatment window. ” It doesn’t really matter if the clot is two, six or eight weeks old, you can still intervene with ClotTriever. If you only had thrombolysis as a treatment option, that would matter and your therapy would fail”.
