LONG-TERM OUTCOMES OF SECONDARY INTRAOCULAR LENS SURGERIES USING THE HAPTIC EXTERNALIZATION SCLERAL SUTURE TECHNIQUE
Bison, Henry S. MD; Thompson, John T. MD
Retina 45(7):p 1332-1338, July 2025
A retrospective study of 223 eyes with IOL fixation using the haptic externalization scleral suture technique. There was a visual acuity improvement from logMAR 1.14 ± 0.05 (20/250-2) preoperatively to 0.48 ± 0.04 (20/63 + 1) postoperatively (P < 0.0001). The rate of IOL dislocation postoperatively requiring reoperation was 9/223(4%). This study demonstrates the long-term success of the haptic externalization scleral suture technique.

This study aimed to evaluate whether OCTA can predict exudative activity changes in nAMD treated with ranibizumab. While OCTA might not forecast imminent exudation, it provides valuable insights into MNV flow. The findings suggest neovascularization remodeling and exudation may represent distinct processes in nAMD.

Real-world adherence to antivascular endothelial growth factor for proliferative diabetic retinopathy is poor: >40% are lost to follow-up within 1 year, and 87% remain nonadherent to the Diabetic Retinopathy Clinical Research Network Protocol S monotherapy regimen of antivascular endothelial growth factor injections.

Ocular toxicity is a concern in eye preservation treatments for retinoblastoma. Our initial investigation focused on the analysis of cytokine levels in the aqueous humor of patients undergoing intravitreal melphalan treatment for retinoblastoma. Elevated cytokine levels after intravitreal melphalan administration may be associated with ocular toxicities.

Rt-PA-ASSISTED VITREORETINAL SURGERY FOR OPEN GLOBE INJURY WITH SUPRACHOROIDAL HEMORRHAGE

Suprachoroidal injection of recombinant tissue plasminogen activator, followed by drainage of suprachoroidal hemorrhage combined with vitreoretinal surgery, can efficiently drain suprachoroidal hemorrhage and restore ocular structure and visual function at an early stage. Retinal loss is the risk factor for poor visual and anatomical outcomes.

The following retrospective analysis found similar clinical outcomes, including the rates of radiation retinopathy, neovascular glaucoma, and enucleation, between conservative versus surgical management of persistent exudative retinal detachments in choroidal melanoma. Refractory exudative retinal detachments were shown to be a significant risk factor for neovascular glaucoma within the cohort.

This study compares endothelial cell loss after pars plana vitrectomy with silicone oil tamponade in rhegmatogenous and tractional retinal detachment. Patients with tractional retinal detachment showed greater loss (9.17% vs. 3.39%). Age, lens status, and combined surgeries were key factors, emphasizing careful monitoring, particularly in older patients with tractional retinal detachment.

A study of 69 eyes from 1,879 cases examined late recurrent retinal detachment, defined as redetachment 3 months ≤ after repair. Despite a 93% reattachment rate, visual outcomes were poor. No significant differences were found between late recurrent retinal detachments after 3 versus 6 months, although later recurrences had worse visual acuity.

In this retrospective multicenter study involving 38 eyes, retinal reattachment after a single pneumatic retinopexy procedure was achieved in 71.1% of vitrectomized eyes, with better outcomes in eyes without a retinal detachment history. Pneumatic retinopexy can be considered a less invasive option for retinal reattachment in select vitrectomized eyes.
