When and How to Apologize?
When you know you need to stay in integrity with yourself and even if you don't know why someone else is upset, you say I am sorry (and then use the words they use such as "you hurt my feelings") that I hurt your feelings. You do not say I am sorry "if" I hurt your feelings. That person has already said -- you DID hurt her feelings. Or you might say: "I get it, I can see why that would upset you. I am sorry.." Don't try an explain then. Or if you feel it is coming out of nowhere...you can say..." I need to know more about that." or "Please tell me more about that."
