Eschar in Rickettsial fever

Patient presented with fever for 8 days,highest recorded temperature 104.5° F. There is also dry cough. Patient is on a third generation IV cephalosporin without any improvement.This simple finding on left upper eyelid gives the diagnostic clue.

A 25 year-old 6+ month pregnant lady presented with anorexia, nausea,vomiting jaundice for the last 15 days. On query she gave history of one miscarriage at 9 weeks age of fetus.She also added that she and recurrent oral ulcer and alopecia. She was diagnosed as hypothyroidism for 1 year and is taking levothyroxine regularly.

Her investigations show high s. bilirubin,SGPT to,SGOT,normal S.ALP.ANA is very high but Anti-dsDNA is negative. Hepatitis B virus and C virus profile is negative.
