👉 Fuel Control Switch Lock Disengagement – FAA SAIB NM-18-33 (December 2018)

Aircraft Affected: Boeing 787 and others
Warning: “Potential disengagement of the fuel control switch locking feature”
The SAIB warned that this locking failure could result in uncommanded movement of the fuel control switches, possibly shutting off fuel without crew intention. Although not an Airworthiness Directive, Boeing operators were advised to inspect the switch mechanisms.
Air India confirmed that VT-ANB had not received or complied with SAIB NM-18-33, since the guidance was advisory, not mandatory.
ANALYSIS:
Given the available facts:
Fuel switches were found in the ON position.
No pilot action was recorded to shut them off.
This strongly supports the FADEC misinterpretation fault, consistent with the ANA case — not manual or accidental movement. The crew were victims of either automated misbehavior (FADEC logic) or a mechanical vulnerability (unlocking switches), both well known to Boeing and regulators.
