‼️Transitioning from children’s to adult services when physical and or medical complex needs are involved‼️

‼️What has your experience been?‼️

It seems that different teams are involved (physical disability team, autism team, learning disability team) depending on a young person’s diagnosis or needs. But when needs overlap, what happens? Which team is responsible?

There is NOT one clear pathway for all young people with complex needs.

🩵 If your young person has physical and/or medical needs, what has your transition into adult services been like?

🩵 If they needed a learning disability diagnosis to access certain services, how did that process work? What happened if they didn’t meet the criteria?

Were you supported, or left to navigate the different teams alone?

I’d love to hear your experiences. Why are pathways so different, and what needs to change? 🩵
