Dr. Vivek explains that the PFA-SHAM Trial also evaluated psychological stress associated with atrial fibrillation, given its known link to increased depression and anxiety. Prior studies, such as the REMEDIAL Trial, suggested that ablation reduces psychological stress more effectively than medication, though those results came from an unblinded study.

In contrast, the PFA-SHAM Trial was blinded and included a pre-specified secondary endpoint assessing changes in psychological well-being using the Hospital Anxiety and Depression Scale (HADS). Results showed a significant improvement in HADS scores in the pulsed field ablation group compared with the sham group, reinforcing that PFA provided benefits across both primary and secondary endpoints.

Anticoagulation and LAAO Insights with Dr. Verma

Dr. Verma emphasizes that fewer than 10% of trial participants had CHA₂DS₂-VASc scores of 4 or higher. For these higher-risk patients, it remains unclear whether anticoagulation can be safely discontinued or must be continued long term, and future trials are needed to answer this question. He also highlights that left atrial appendage occlusion appears to have no role in lower-risk patients based on the trial’s findings, though its potential relevance in higher-risk groups remains uncertain.

PFA-SHAM Trial Interim Results with Dr. Vivek

Dr. Vivek reports that the PFA-SHAM Trial met both co-primary endpoints at the 50-patient interim analysis. The study demonstrated strong safety outcomes, with no major complications and full six-month follow-up for all participants. A small number of patients crossed over from the sham to the ablation group under predefined clinical criteria.
