Requirements for Hospital Bills Submission:
1. Medical Certificate
2. Medical Abstract
3. Quotation (if not yet admitted)
4. Running Bill (for in-patient expenses)
5. Promissory Note (if discharged)
6. Final Bill
7. Social Case Study (from Municipal Hall/City Hall) – valid for 3 months
8. Certificate of Indigency (from Barangay Hall) – valid for 45 days
9. Letter addressed to the corresponding public official
10. Valid ID's (patient/representative)

Requirements for Medicines Submission:
1. Medical Certificate
2. Medical Prescription
3. Price Quotation
4. Social Case Study (from Municipal Hall/City Hall) – valid for 3 months
5. Certificate of Indigency (from Barangay Hall) – valid for 45 days
6. Letter addressed to the corresponding public official
7. Valid ID's (patient/representative)
