What happens when cabin crew asks for medical assistance onboard?

Medical situations happen on flights more often than most people realize. While cabin crew are trained to provide first aid and manage a wide range of onboard medical emergencies, some situations require additional medical expertise.

The medical equipment available depends on the airline and the regulations it operates under. Some aircraft carry basic first aid supplies, while others, especially those operating long-haul and ultra-long-haul international flights, carry more extensive medical kits, emergency medications, and equipment.

If extra assistance is needed, cabin crew may make an announcement asking if there is a doctor or other licensed medical professional onboard. Depending on the airline’s policies, they may be looking for:

* Doctors
* Registered nurses
* Paramedics
* Midwives
* Other licensed healthcare professionals

In many cases, the airline will ask the volunteer to provide proof of their medical credentials before allowing them to assist. If their qualifications cannot be verified, the airline may decide not to accept their assistance, depending on company policy and the circumstances.

Can constant flying affect a flight attendant’s body and mind?

Most people don’t think about it, but cabin crew expose themselves to conditions that are very different from normal life.

⭕️ Radiation - At cruising altitude, the atmosphere provides less protection from cosmic radiation. Long-haul and polar routes, like Dubai–Los Angeles, can expose crew to radiation levels comparable to a chest X-ray. In some countries, this is recognized as occupational exposure, and airlines monitor crew radiation exposure. Studies have also found that flight attendants have higher rates of some cancers, although researchers believe multiple factors (including radiation and disrupted sleep) likely contribute.

⭕️ Irregular sleep - Night flights, early reports, and constant time zone changes disrupt the circadian rhythm and melatonin production, making proper recovery sleep more difficult.

⭕️ Fatigue - Long duties, sometimes exceeding 16 hours, combined with jet lag and reduced rest can affect reaction time, concentration, mood, and long-term recovery.

⭕️ Cabin pressure - Aircraft cabins are pressurized to the equivalent of about 6,000-8,000 feet above sea level. Lower oxygen levels can contribute to tiredness, headaches, and reduced physical performance.

⭕️ Dehydration - Cabin air is extremely dry, which can lead to dry skin, dry eyes, headaches, and fatigue.

⭕️ Mental health - Constant travel can also feel isolating. Cabin crew often miss birthdays, weddings, holidays, and important family moments. Add difficult passengers, emotional labour, lack of routine, and being “always on,” and the job can become mentally exhausting too.

Being cabin crew is an incredible career, but it comes with physical and emotional costs that passengers rarely see.

Why can’t cabin crew just move you to Business Class if the seat is empty?

Because it’s not just the seat to give away. A few things passengers often don’t realize:

- Crew must follow company policy. On many airlines, giving away a premium seat without authorization can lead to disciplinary action and, in some cases, even termination.

- Empty Business Class seats are often kept available for paid upgrades during the flight. Passengers do purchase upgrades onboard more often than you might think.

- An upgrade is not just a different seat. It includes the full Business Class service: meals, drinks, amenities, etc, and documentation that must be accounted for.

- In certain medical situations, premium cabin seats may be needed to accommodate a passenger who cannot remain seated upright in Economy.

- Just like you can’t give away your company’s products or services for free without permission, cabin crew can’t give away Business Class seats simply because they happen to be empty.

Think of it this way:

You wouldn’t walk into a hotel and say:
“Do you have an empty suite? Great. Put me there for free. It’s empty anyway.”

The seat may be empty, but it still belongs to the airline.
