Be very careful in management of T2DM in elderly.as we know all target blood sugar is f less than equal to 100,pp less than equal to 140,in the elderly we keep the blood sugar a bit high.fssting around 120 and pp around 170,cause being if hypoglycemia sets in in the elderly it is dangerous as the counter regularly hormone like adrenaline,not adrenaline break down body fats and try to provide more sugar to the body and these counter regulatory hormones may produce fatal myocardial infarct or acs.

The newer antidiabetic drug few are secretogauge,like GLP 1,gip,dpp4 inhibitors and few sensitiser like SGLT2 inhibitors,and other linagliptin, having work on hepatic ribosomes are now in the pen of diabetologist .but the real prescription should authenticate its use and how it becomes superior to less costly drugs specially glimiprimide the newer generation secretogouge having glucose sparing effect and using alternate atpase na.k channel and metformin big sensitiser,blocking reverse lipid transport should be our main stay unless there is compelling indication as last two drugs are reopened in 1923 -24 IDF summit and several other conditions
