PSUEDO-MΥΟΡΙΑ v/s DEPRESSION

Today, I encountered a 15-year-old patient with a complaint of blurred vision while studying. His initial auto-refraction values were -7.5 diopters sphere in the right eye and -6.75 diopters sphere in the left eye. His visual acuity was approximately 6/12 partial.

After dilating his eyes with cyclopentolate for cycloplegic refraction and waiting 1.5 hours, his auto-refraction values changed to 0.0 diopters in both eyes. This indicated pseudo-myopia.

Upon taking a detailed history, several factors emerged:

1. He does not have excessive screen time.

2. He does not study excessively.

3. He does not use any medication for any disease.

However, he was accompanied by an attendant (not a family member) who mentioned that his parents are at the brink of divorce, fighting 24/7, causing the patient severe depression. Consequently, he avoids school, sleeps all day, uses screens during his free time, and does not socialize with friends.

Based on this, I advised the attendant to change the patient's routine and referred him to a psychologist for counseling. No glasses or medication were needed, just supplements and psychological support.

Nowadays, pseudo-myopia is common among adults and children due to financial issues, family problems, and other stressors, often leading to severe depression. It is crucial for optometrists to detect pseudo-myopia and recommend cycloplegic refraction. After this evaluation, patients should be referred to a psychologist for appropriate counseling. It is our moral responsibility to address not only the optical but also the psychological well-being of our patients.
