Not all leg swellings are what they seem. When a 55‑year‑old cancer patient presented with persistent unilateral leg swelling, the initial diagnoses—deep vein thrombosis, cellulitis, and Baker’s cyst—were quickly ruled out. What followed was a complex diagnostic journey that ultimately revealed a rare cause: proximal venous obstruction secondary to diffuse peritoneal carcinomatosis.

This recently published case report on PoEM demonstrates how a multidisciplinary, imaging‑led strategy can unmask atypical threats that elude conventional workups. This case reinforces the need to think beyond textbook patterns—especially in oncology patients—so that life‑threatening diagnoses aren’t missed.
