INJECTION TECHNIQUE
Intramuscular (IM) injection is a common and important nursing skill used to deliver medication deep into a muscle so it can be absorbed quickly into the bloodstream. Here’s a clear, student-friendly breakdown 👇
💉 Intramuscular (IM) Injection Technique
1. Preparation
√Wash hands properly 🧼
√ Gather equipment:
√ Sterile syringe and needle
√ Medication
√ Alcohol swab
√ Cotton/gauze
Check the 5 rights:
° Right patient
° Right drug
° Right dose
° Right route
° Right time
2. Select the Injection Site
Common IM sites:
• Deltoid muscle (upper arm) – small doses
• Vastus lateralis (thigh) – safe for all ages
• Ventrogluteal (hip) – safest for adults
• Dorsogluteal (buttock) – less preferred (risk of nerve injury)
3. Position the Patient
√ Make the patient comfortable
√ Expose the site
√ Relax the muscle (very important to reduce pain)
4. Clean the Site
° Use an alcohol swab
° Clean in a circular motion (center → outward)
° Allow to dry (do not blow or fan)
5. Injection Procedure
• Hold syringe like a dart
• Stretch the skin (or use Z-track technique if needed)
• Insert needle at 90° angle straight into the muscle
• Aspirate (depending on protocol — many guidelines now say no aspiration for vaccines).
• Inject medication slowly and steadily
6. After Injection
√ Withdraw needle quickly
√ Apply gentle pressure with cotton/gauze
√ Do not massage (especially for some medications)
√ Dispose of needle safely in a sharps container
7. Documentation
• Record:
• Drug name
• Dose
•Site
•Time
• Patient response
