The man sitting at the front of Alaafin is Ọtun Ẹfa. He's unarguably the closest of the Ẹmẹwas to the Alaafin, not the head of the Ẹmẹwas though. He must be everywhere with Alaafin.

If he enters A*o Rock, he should be there. He must be with him in the bedroom. He will be at the entrance while Baba is inside with his wife. If Baba is strong, he will be sitting beside the hospital bed space. If Aláàfin's wife/wives are denied accessibility due to one reason or the other as it may happen, Ọtun Ẹfa must be with him.
