PURPOSE OF HAND SPLINT FOR CHILDREN WITH CEREBRAL PALSY

• Wrist and hand splints are provided to people who need protection and support to reduce further tightness around the joints of the wrist and fingers or weak joints and their surrounding structures. Their designs make sure you position your wrist and hands correctly.

• The splint holds the joint in a neutral position. When the symptoms are worse at night because your hand is more likely to bend while you're sleeping, splints become very essential tools to maintain the range of motion of joints so that further deformities can be prevented. The splint prevents this from happening.

• It's also important to move the joint normally during the day to keep it from becoming stiff and stop the muscles from becoming weak and atrophy.

• The main function of a hand splint is to reduce stiffness in the joints of the wrist and hand, reduce pain in the wrist and hand, keep the wrist and hand in the correct position, reduce swelling and morning stiffness. For increases the strength of your fingers etc.

Principles of hand splinting:
• Ligaments of digit should be maintained
• Arches of the hand should be pressed
• Dual obliquity of the hand should be maintained
• Bony prominence should be avoided
• Wrist should be splinted in slight extension to prevent the non-functional position of the wrist and subsequent potential deformity from developing
• Length of collateral ligaments should be maintained in a splint
• Fibrous structure and blood supply should be considered properly
• To avoid skin pressure the area of splint application should be wide enough to disperse the pressure
• The discomfort of perspiration can be alleviated by absorbent socks such as stockinet tubing over the part

Sports injuries refer to the kinds of injury that occur during sports or exercise. While it is possible to injure any part of the body when playing sports, the term sports injuries is commonly used to refer to injuries of the bones and joints. Obviously, football is the commonest sports in Nigeria, so we expect more injuries because it involves the knee, ankle and back. Orthotic braces are medical devices designed to address musculoskeletal issues they are used to properly align, correct the position, support, stabilize, and protect certain parts of the body (particularly the muscles, joints, and bones) as they heal from injury or trauma.

There are 3 main uses of bracing in sports injuries-
►Prevention: Aid in stabilization, support, and protection of uninjured or fully rehabilitated joints and soft tissues.
►Treatment: Minimize pain and swelling in the acute phase, aid in unloading painful structures, and provides support to unstable joints.
►Rehabilitation: Aid in early mobilization, muscle imbalances, neural control, proprioception, and protect healing injuries.

PRINCIPLES OF BRACING
●Bracing material should be functional during sport and daily activities, comfortable, lightweight, and moldable braces enhance their functionality.
●Braces should fit appropriately and be adjusted as swelling improves and activity progresses.
●Braces may migrate during activity. Tape, straps, and undergarments may be used to decrease unwanted migration.
●Brace evaluation over time is necessary to ensure appropriate function and to assess need for part replacement.
●Over-the-counter braces are instantly available, less expensive and efficient to use.
●Custom applications constructed from thermoplastic and other materials available for specific athlete needs (e.g. clamshell brace or ankle foot orthosis.)

What is rehabilitation?
Rehabilitation is care that can help you get back, keep, or improve abilities that you need for daily life. These abilities may be physical, mental, and/or cognitive (thinking and learning). You may have lost them because of a disease or injury, or as a side effect from a medical treatment. Rehabilitation can improve your daily life and functioning.

Who needs rehabilitation?
Rehabilitation is for people who have lost abilities that they need for daily life.

Some of the most common causes include

Injuries and trauma, including burns, fractures (broken bones), traumatic brain injury, and spinal cord injuries
Stroke
Severe infections
Major surgery
Side effects from medical treatments, such as from cancer treatments
Certain birth defects and genetic disorders
Developmental disabilities
Chronic pain, including back and neck pain

What are the goals of rehabilitation?
The overall goal of rehabilitation is to help you get your abilities back and regain independence. But the specific goals are different for each person. They depend on what caused the problem, whether the cause is ongoing or temporary, which abilities you lost, and how severe the problem is.

For example, A person who has had a stroke may need rehabilitation to be able to dress or bathe without help

An active person who has had a heart attack may go through cardiac rehabilitation to try to return to exercising

Someone with a lung disease may get pulmonary rehabilitation to be able to breathe better and improve their quality of life

What happens in a rehabilitation program?
When you get rehabilitation, you often have a team of different health care providers helping you. They will work with you to figure out your needs, goals, and treatment plan. The types of treatments that may be in a treatment plan include

Assistive devices, which are tools, equipment, and products that help people with disabilities move and function

Cognitive rehabilitation therapy to help you relearn or improve skills such as thinking, learning, memory, planning, and decision making

Mental health counseling

Music or art therapy to help you express your feelings, improve your thinking, and develop social connections

Nutritional counseling

Occupational therapy to help you with your daily activities

Physical therapy to help your strength, mobility, and fitness

Recreational therapy to improve your emotional well-being through arts and crafts, games, relaxation training, and animal-assisted therapy

Speech-language therapy to help with speaking, understanding, reading, writing and swallowing

Treatment for pain

Vocational rehabilitation to help you build skills for going to school or working at a job

Depending on your needs, you may have rehabilitation in the providers' offices, a hospital, or an inpatient rehabilitation center. In some cases, a provider may come to your home. If you get care in your home, you will need to have family members or friends who can come and help with your rehabilitation.
