A real but very rare medical phenomenon known as defecation syncope.
When a person strains excessively during a bowel movement, they perform what's called a Valsalva maneuver, holding their breath and pushing hard.
This increases pressure inside the chest and stimulates the vagus nerve, which helps regulate heart rate and blood pressure.
In some people, especially older adults or those with underlying heart conditions, this can cause the heart rate and blood pressure to drop suddenly.
The result may be dizziness, fainting, or loss of consciousness while on the toilet. In extremely rare cases, the sudden cardiovascular changes can trigger dangerous heart rhythms, cardiac arrest, or serious injuries from falling after fainting.
While headlines often say people can "die from p**ping too hard," the bowel movement itself isn't usually the direct cause. Rather, it's the body's abnormal response to intense straining.
