2. Updated PRC license (photocopy);

5. Professional Tax Receipt (PTR-photocopy);
6. Sample of Job Certification;
7. BIR Registration (photocopy);
8. Calling Card;
9. Rubber Stamp with specimen signature;
10. Office Picture with signboard;
11. Location Sketch of Business Office;
12. Sample copy of Official Receipt;
13. Updated 2x2 Passport I.D. Picture;
14. Filled-up AEP Information Sheet;
15. Filled-up Memorandum of Agreement.

For those who are renewing their accreditation with BENECO as Accredited Electrical Practitioners, the following are the grounds for automatic denial and/or revocation of accreditation, to wit:

a. Accredited Electrical Practitioners still with Pending Application for Service Connection (ASC) due to deficiencies on metering or non-compliance to the provision of the latest edition of the PEC and BENECO standards;

b. Accredited Electrical Practitioners who submitted falsified documents as attachments to the Application of Service Connection and/or falsification of the application itself; and

c. Expired PRC licenses.
