What Happens to the Ovaries in PCOS?

The image shows the female reproductive system with a polycystic o***y. In Polycystic O***y Syndrome (PCOS), the ovaries look different from normal ovaries because they contain many small fluid-filled follicles.

1. Multiple Small Follicles (Often Called “Cysts”)

In PCOS, the o***y contains many immature follicles that look like small cysts.
These follicles contain eggs that did not mature or release properly.

2. Ovulation Does Not Occur Normally

Normally, one follicle grows and releases an egg during ovulation.
In PCOS, the follicles stop developing before ovulation, so the egg is not released.

This can cause:
• Irregular menstrual cycles
• Missed periods
• Difficulty getting pregnant

3. Enlarged O***y

Because many follicles remain in the o***y, the o***y may become slightly enlarged compared to a normal o***y.

4. Hormonal Imbalance

PCOS is also linked to higher levels of androgens (male hormones).
This hormonal imbalance contributes to symptoms like:
• Acne
• Excess facial or body hair
• Hair thinning on the scalp
• Irregular periods

5. “String of Pearls” Appearance

On ultrasound, the small follicles often line up around the o***y, creating what doctors call a “string of pearls” pattern.
