AZITHROMYCIN: SOBRANG TAPANG BA TALAGA?

🤔"Niresetahan ang anak ko ng 3 days antibiotics na azithromycin. Siguro super tapang na antibiotics ito kasi mahal and 3 days lang unlike mga dating nirereseta na 7 days???"

🤓1. hindi all brands ng azithro mahal

- Iyong innovator brand [original version; the first approved brand] ay 600+ pero 300+ to 400+ lang naman most other brands.

🤓2. long half-life hindi tapang

- HINDI tapang or lakas ang dahilan why it can be given once a day for few days only. Long half-life ang dahilan pero ano ba ang half-life?
- Look at the attached photo. Para po siyang phone na matagal malowbatt 😅 May mga antibiotics na after inumin, after a few hours wala na ulit sa katawan kaya need inumin so frequently. Kung matagal malowbatt hindi need charge nang charge 😅 ... so hindi need paulit ulit uminom ng azithro.
- It does NOT only work during the number of days na ininom iyan. Giving it for shorter days does NOT mean mas mabilis siya makacure than the antibiotics na mas matagal need inumin.

💀👻DANGER SIGNS and SYMPTOMS SERIES💀👻

FEVER PLUS...

😑"Kapag maaga mo dinala sa ER ang batang may fever, hindi tinatanggap or pinapauwi din agad after quick check up. Kung naghintay naman ng several days bago dinala, papagalitan naman bakit late dinala. So saan dapat lulugar ang mga magulang?"

🤓IT DEPENDS! 😉Many illnesses na may fever ay hindi tunay na emergency. HINDI kasi tagal ng fever ang basis ng pagdala ng bata sa ER kundi ang mga findings na kasabay ng fever hence the title ng post na ito "FEVER PLUS". Maawa po kayo sa mga frontliners and mas malalang patients. Huwag po dalhin sa ER ang mga hindi naman tunay na emergencies!!!

😱looks VERY ill, is unusually drowsy, or is very fussy
- May mga karamdaman tulad ng meningococcemia na puwedeng mamatay ang pasyente within 24 hours lang! Therefore do not wait 3 days bago ipa ER kung nakita niyo namang hinang hina na ang pasyente!

😱has been in a very hot place such as an overheated car

- ex. Gusto mong ipasyal ang baby pero ECQ pa so sinama mo sa kotse... tapos may bibilhin ka so iniwan mo saglit ang baby at pinatay ang makina. Pagbalik mo mainit na ang kotse and ang baby. Aba huwag na mag-isip ng pagngingipin, pahilot baka pilay, etc. Go to ER.

😱has other symptoms, such as a stiff neck, severe headache , severe sore throat, severe ear pain, an unexplained rash, or repeated vomiting or diarrhea

- in short obviously kaconfine confine na ang bata so huwag nang dalhin kung saan saan; hindi na iyan online case

😱has immune system problems, such as sickle cell disease or cancer, or is taking steroids

- madaling lumala kasi nga may problem ang immune system so dapat nachecheck agad

😱Has had a seizure

😱Is younger than 3 months (12 weeks) and has a temperature of 100.4°F (38.0°C) or higher

- Lagi nating iisipin ang posibilidad ng sepsis sa younger than 3 months na may lagnat... puwedeng ikamatay ang sepsis so emergency iyan.

😱Fever rises above 104°F (40°C) repeatedly for a child of any age

👨🏻‍⚕️ sabi ng doctor pumunta kayo sa ER

- Wala ito sa original list na gawa ng American Academy of Pediatrics. However, general guide lang siyempre ang mga nasa list. Iba pa rin ang medical opinion ng inyong doctor (unless kaya ka lang niya pinapa ER kasi wala siyang online clinic and ayaw ka niyang macheck up ng ibang pedia online).
-In addition, diba sinabi ko ngang hindi safe sa physical clinic ang fever kasi baka makahawa kaso may fever na need talaga icheck physically 🙁bawal sa physical clinic bawal sa online so ayun siyempre ER na ang isusuggest sa inyo.
ex. some cases ng fever na hindi nawala despite giving medicine na appropriate for the case

PAGLILINAW in case may mga mali mali ang intindi: Ang topic ng post ay some clues na need dalhin sa ER ang patient na may fever. IBANG issue iyong when to do non-ER consultation for fever. HINDI dahil walang nameet na criteria above ay ibig sabihin hindi need ng check up.

CEFUROXIME VS CEFACLOR

🤔"May masarap naman palang antibiotics, iyong cefaclor. How come binebenta pa rin ang mga antibiotics na masama ang lasa like cefuroxime???"

🤓"Magkamag-anak" na antibiotics ang cefuroxime at cefaclor. Same lahi/klase sila ng antibiotics.

❤️Walang dudang mas masarap ang cefaclor kaysa sa cefuroxime. However, iyon lang talaga ang benefit niya over cefuroxime...

🖤Naalala niyo ang discussion ko about beta-lactamase last time? Para siyang weapon ng bacteria against antibiotics. It is a common misconception na dahil same type of antibiotics sila ay equally effective sila. That is NOT true. Although theoretically they are supposed to work against the same type of germs, mas mahina ang cefaclor against beta-lactamase... meaning cefuroxime may higher chance to survive against bacteria with weapon vs cefaclor na patay agad kapag may weapon si bacteria.
🖤Although hindi common, cefaclor can cause serum sickness-like reaction [fever, pantal, joint pain, etc]. Cefuroxime ay walang ganyang side effect.
🖤WALA pong infection na cefaclor ang preferred choice of antibiotics kahit sa guidelines ng mga sakit or even sa National Antibiotics Guidelines ng Pinas.

👨🏻‍⚕️Hindi naman po banned antibiotics ang cefaclor so we can still use it once in a while. However, due to the reasons mentioned above, hindi po natin dapat maging favorite antibiotics ang cefaclor... ✌🏻 Hindi po natin siya dapat first choice in most patients ...

Hindi ko pa alam ang sched ko Thursday Friday. Yesterday afternoon ko lang kasi nalaman na sarado clinics ko on Saturday [dahil sarado sila not because ako ang nagdecide na hindi pumasok]. Parang gusto ko pumunta somewhere pero ang iksi na ng time to plan anywhere na malayo 😅 abang na lang new announcement kapag naisip ko na....

🤓Oct 27 Monday
- Casa Medica SM Southmall 12:15-1:30 PM and 4-5 PM... 5 ang alis hindi 5 darating pa lang kayo.

💻no more online clinic on Mondays as announced several days ago

🤓Oct 28 Tuesday
- no physical clinic
💻8-9 AM PAID online clinic may bayad hindi free reseta kahit dati kayong pasyente.... private pedia po ako

🤓Oct 29 Wednesday
- Casa Medica 10 AM-1 PM arrive before 12:30 PM

❤best day for new well child visits (transfer from another doctor) and concerns na mahaba haba
💉best day for vaccination; PM 2-3 days before para magpasched

👨🏻‍⚕Non-vaccination CHECK UP is by walk-in first come first served. NO need to PM na pupunta kayo. Concern/s na need ng gamot ay need ipacheck up at hindi PM PM lang.
🩺FOLLOW UP ay physical NOT online unless otherwise mentioned during the previous check up. Persistence of old symptoms and/or appearance of new symptoms require physical follow up not online tanong tanong.
💉VACCINATIONS by appointment not by walk-in... PM 2-3 days before planong araw para maready ko ang vaccines. Cash and gcash accepted. No credit card no bank to bank transfer. Ideal date for vaccination ay Wednesday Casa and Saturday HP.
🏥For admission/CONFINEMENT, go to Las Pinas City Medical Center ER (City Med; HMOs accepted) or Las Pinas Doctors Hospital ER (LPDH; HMOs accepted) .
💤REST TIME 10 PM-8 AM do not PM regardless kung ano ang concern. I am NOT available 24/7. If totoong urgent, bukas ang ER 24/7... we are humans too na kailangan ng rest. HINDI facebook ang tamang lugar for emergencies.

❌NO mix and match po. If child is sick (ubo sipon, diarrhea, etc. , then sickness po ang ichecheck and NOT included ang well child issues (vitamins, deworming, screening tests, etc), and developmental issues (delayed speech, delayed sitting, etc.). Multiple symptoms of related problem (ubo + sipon) sige lang pero multiple types of check up (symptoms of sakit + development issue) ay no no lalo kung hindi ko regular patient...
❌not accepting newborns less than 1 month old... sa ibang pedia niyo ipa"catch" ang baby during panganganak at sa iba niyo ipacheck before 1 month old

Antivirus sticker? Anti-bacterial sticker? Wow magic??? 🙄😆 Mawawalan na ba ng patients ang mga pediatric infectious disease specialist??! Siyempre hindi. 😆 I hope wala akong followers na naniniwala sa ganyan dahil kung meron OMG mukhang wala kayong natutunan sakin 😆

Kung ganyan kadali pumasok ang antivirus at antibiotics sa katawan e di sana di na naimbento ang syrups at injectable meds 🤪🤡

👨🏻‍⚕Buy face masks, soap, healthy food. Get vaccinated. Sleep well. Take food supplements if deemed necessary/appropriate by a doctor.

FYI, ang decision to admit ay based sa paglabas ng warning signs or symptoms at usually HINDI sa specific name ng sakit....

FYI, ang decision to admit ay based sa paglabas ng warning signs or symptoms at usually HINDI sa specific name ng sakit. Mahalaga ring malaman eventually ang sakit pero kung may serious problems na kaya nang matreat even without specific diagnosis, then galaw galaw na❗

Treat the Whole Patient Not the Test Results Only

🤓QOTD: "Wala pang tests admit agad? Baka mukhang pera lang ang doctor?"
🤓"Bakit aadmitin kung okay ang test results?"

🖤Kung hingal na hingal ang pasyente and normal ang CBC, does it mean imagination lang ng patient ang symptoms? No. Patient can have severe pneumonia and normal blood tests.
🖤Kung normal ang fecalysis pero matindi na dehydration ng bata, does it mean guni guni lang ng doctor ang dehydration? No. Hindi naman nakikita sa fecalysis ang hydration level. 😱😮🤯 Ito ang nakakaworry minsan sa ibang parents. May mga parents na sariling nagkaclassify ng lala ng anak based only on results.
🖤Kapag hingal na hingal ang patient pero clear lungs and normal chest x-ray, does it mean nag-iinarte lang ang patient? Usually hindi. Baka mamiss ang diabetic ketoacidosis kung we will rely on chest x-ray sa lahat ng hirap huminga.

👨🏻‍⚕️ May mga cases na hindi very clear from interview and check up kung tunay na worrisome ang cause ng seemingly dangerous signs or symptoms. That is the time na doctor may wait for test results.
ex. sobrang sakit ng tiyan according to patient but physical exam by pedia and surgeon sa ER seems fine -> ending minsan constipation lang 😱 and kayang masend home
👨🏻‍⚕️Iba iba ang purpose ng tests. May pangconfirm, may pangclassify ng lala, may pangcheck ng cause, etc. Kami pong doctors ang nakakaalam which is which. If alam naming the results will not change our decision to admit, then we can admit/confine the patient kahit wala pang any tests at all.

CEFALEXIN: NOT FOR PNEUMONIA

🤔"Ang anak ko ay niresetahan ng cefalexin para raw sa pulmonya. Hindi tumalab kaya nagpasecond opinion kami. Binigyan siya ng ibang antibiotics kasi para sa skin infection daw ang cefalexin not for pneumonia."

🤓FACT: Lahat ng naabutan kong guidelines for treatment ng pneumonia sa Filipino children mula 2004 hanggang sa latest edition ay HINDI kasama ang cefalexin sa choices. These guidelines ay downloadable for free sa internet so you can fact-check.
🤓FACT: Sa mga naag-iisip na baka hindi best choice kaya wala sa list pero acceptable pa rin gamitin.... unfortunately, talagang no... sabi ng feigin and cherry's textbook of pediatric infectious diseases "Because 1st gen ceph have minimal, if any, activity against Haemophilus influenza and M. catarrhalis , they shoud NOT be used for empiric treatment of respiratory tract infections."

🖤Sa mga followers kong doctors, some of you may think "bakit need pa pag-usapan ito hindi ba common sense na hindi naman talaga for pneumonia ang cefalexin???" Unfortunately, hanggang 2025 may mga nagpapasecond opinion pa rin dahil hindi gumaling after receiving cefalexin. 😞

AMOXICILLIN VS
CO-AMOXICLAV (AMOXICILLIN AND CLAVULANIC ACID)

🤔"Ang anak ko ay niresetahan ng amoxicillin last month dahil sa bacterial tonsillitis tapos ngayon co-amoxiclav naman for bacterial pneumonia. Dalawang antibiotics ba ang amoxillin saka clavulanic acid? Bakit hindi amoxicillin na lang ulit para mas mura."

🤓Tingnan ang attached photo para mas madaling maintindihan ang clavulanic acid.
🖤Marami ang klase ng mga bacteria. Ang iba ay mas malakas tulad ng bacteria sa picture na may sword called beta-lactamase na kayang sirain/patayin ang antibiotics.
❤️Tingnan naman ang left side ng photo. Clavulanic acid is a type of beta-lactamase inhibitor. Isipin niyo na para siyang shield na panlaban sa sword ng bacteria. Clavulanic acid protects the amoxicillin.

👨🏻‍⚕️HINDI dalawang antibiotics ang amoxillin saka clavulanic acid. Amoxicillin lang din ang antibiotics in co-amoxiclav.
