2️⃣ Fill out all required information completely.
3️⃣ Return the completed Conforme to the Publications Office.

▪️Have a recent 1x1 photo ready and a photocopy of your PSA birth certificate.

▪️Check your PATTS email for a confirmation – this confirms your Conforme has been processed.

- Fill it out.
- Submit it to Contrails in the Publications Office during duty hours.
