Accomplished Application Form
Original and photocopy of Philippine Statistics Authority (PSA) issued Certificate of Live Birth on Security Paper; and
Married Females (who are using their spouse’s last name) must also present Original and submit photocopy of PSA-issued Certificate of Marriage on Security Paper or Report of Marriage.
Local Civil Registrar Copy is required if PSA-issued documents are not clear or cannot be read.
Any of the following acceptable IDs with one (1) photocopy

Note: If a woman opts to retain maiden name, a PSA-issued Certificate of Marriage is not required.
Supporting Documents for Adult New Applications
Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant’s identity, citizenship and lack of travel restrictions, as specified in the following cases:

If you do not have a PSA-issued birth certificate, you can use a certified true copy of your Local Civil Registrar (LCR) birth certificate, or if born abroad, your original Report of Birth. You can also order a PSA-issued copy online through services like psahelpline.ph or psaserbilis.com.ph. If the PSA-issued document is a late registration, you will need to provide additional supporting documents like school records, a baby book, or valid government-issued IDs.
If you don't have a PSA birth certificate
Use an alternative: You can use a certified true copy of your local Civil Registrar (LCR) birth certificate as a substitute.
If born abroad: Submit the original Report of Birth or a first indorsement from the Consular Records Division (CRD).
Order one online: You can order an official PSA copy through online services like PSAhelpline.ph or psaserbilis.com.ph.
If you have a late registered birth certificate: You may need to provide additional supporting documents. These can include government-issued IDs, school records, or a baby book.
If you were born before 1950: You will need to submit an original Certificate of No Birth Record from the PSA along with an Affidavit of Two Disinterested Persons.
Additional requirements
For late registrations: If your PSA-issued birth certificate was registered late, you must provide at least one of the following supporting documents:
School records
Baby book or health record
Any two of the following: NBI Clearance, Member Data Record from PhilHealth, or School records like Transcript of Records or Diploma
Other requirements: You may also need other documents depending on your situation, such as a marriage contract or a government-issued ID.
How to order a PSA birth certificate
Go to psahelpline.ph or psaserbilis.com.ph.
Order your birth certificate online and follow the instructions for delivery. You can also use the service to order a Report of Birth.
Wait for the certificate to be delivered to your address. You may need to provide your local Civil Registrar (LCR) copy if the PSA-issued document is not clear or cannot be read.
Provide the PSA-issued certificate to the passport office for processing.

If your passport is mutilated or damaged, you must treat it as a new application and apply for a replacement at a Department of Foreign Affairs (DFA) office. Requirements include securing an online appointment, appearing in person, submitting a duly accomplished application form, your latest passport (if available), and an Affidavit of Damage/Mutilation. Be prepared for additional supporting documents to be requested to verify your identity and citizenship.
Requirements for replacement
Online appointment: Book an appointment through the DFA Global Online Appointment System at passport.gov.ph.
Personal appearance: You must appear in person at the DFA office on your appointment date.
Application form: Complete a duly accomplished application form.
Most recent passport: Provide your most recent passport, if available, along with a photocopy of the data page.
Affidavit of Damage/Mutilation: You may need to submit a written statement explaining the damage. Some embassies may have a specific Affidavit of Mutilation form.
Birth certificate: A Philippine Statistics Authority (PSA)-issued Certificate of Live Birth is often required, as a damaged passport is treated like a new application.
Other documents: Be ready to submit additional supporting documents to verify your identity and citizenship, such as other official identification documents.
Police report: A police report may be required in some cases.
Important considerations
Treat as a new application: A damaged passport is treated as a new application, so you must comply with all requirements for a new passport.
Supporting documents: The Philippine Consulate reserves the right to require more documents if the damage makes it difficult to verify your identity and personal particulars.
Unauthorized changes: Do not make unauthorized changes to your passport, as this will invalidate it and may lead to legal penalties.

Annotated Marriage Contract to show annulment/divorce/court ordered instruction
Death Certificate of Spouse


NOTE: If woman opts to retain maiden name, a Marriage Contract is not required.
