- SF 9 (Report Card) with Second Quarter grades (if available)
- Certificate of Good Moral Character
- Photocopy of PSA Birth Certificate
- Photocopy of Baptismal Certificate
- Two (2) pieces 1×1 ID pictures
- Two (2) pieces 2×2 ID pictures
- Certificate of Completion (for Grade 11)

GRADE 11 – NO TUITION FEE (for public school and ESC Grantee from Grade 7 to 10).

For transferees from Non-ESC Private School: submit documents to be applied for Online Voucher Application Portal (OVAP).

NOTE: SHS waiting for DepEd announcement on Strengthened SHS Implementation.
