Position: Sr. Supervisor – Chemical Pathology
Location: Dr. Ziauddin Hospital (Head Office, North Nazimabad, Karachi)
Academic Qualifications: BS in Medical Technology (MLT) / BSc. or MSc. in Biochemistry / Chemical Pathology field.
Experience Required: Minimum 12 years of experience in clinical chemistry (with at least 2 years in a supervisory role).
🛠️ Key Responsibilities:
Overseeing daily operations of the Chemical Pathology section and ensuring accurate testing workflows.
Managing inventory, analytical platforms, validation, and calibration.
Leading the section through CAP and ISO 15189 accreditation audits.
Managing staff rosters, technical training, and resolving day-to-day lab issues.

How to interpret TFTs step by step 🧵

Example:

TSH = 0.001
Free T3 = 56.8
Free T4 = 7.40

1️⃣ Start with TSH first.
TSH is almost undetectable here.

A very low TSH usually means the body is trying to suppress an overactive thyroid.

2️⃣ Then check Free T4.
Free T4 is markedly elevated (7.40).

Low TSH + High T4 strongly points toward primary hyperthyroidism.

3️⃣ Now look at Free T3.
Free T3 is also very high (56.8).

This explains symptoms like:
• Palpitations
• Tremors
• Sweating
• Weight loss
• Anxiety

4️⃣ Put everything together.
Low TSH + High T3 + High T4 = Thyrotoxicosis / Primary hyperthyroidism

Most likely causes include Graves’ disease, toxic multinodular goiter, or thyroiditis.

TFT interpretation becomes easy when you read it in sequence instead of randomly looking at.

“High TLC = infection” ❌
This is one of the most common clinical shortcuts… and also one of the most misleading ones.

Leukocytosis is not a diagnosis — it’s a signal 📊
A signal that something is happening in the body… but not always infection.

Yes, infection is a common cause 🦠
But not the only one.

Think beyond infection 👇

🧠 Physiological stress response
Trauma, surgery, seizures, severe pain, MI — all can push TLC up via catecholamine-driven demargination.

💊 Drugs (especially steroids)
A single dose of corticosteroid can raise TLC significantly — not by infection, but by shifting neutrophils into circulation.

🔥 Inflammatory & metabolic states
DKA, pancreatitis, burns, pulmonary embolism — all can present with leukocytosis without infection.

🩸 Hematological causes
Leukemias and myeloproliferative disorders — where high TLC reflects production, not response.

🚬 Others
Smoking, emotional stress, even intense physical exertion.

So the real clinical question is not:
👉 “Is TLC high?”
But:
👉 “What is driving it in this patient?”

CBC is not a final answer ❌
It’s a clue sheet 📄

And TLC alone should never label a patient as “infected” without:
✔ Clinical correlation
✔ Differential count
✔ Peripheral smear review
✔ Overall context

Because in medicine…
numbers don’t treat patients — clinicians do. 🩺
