How Glaucoma Damages The Eye?

Glaucoma is an eye condition that develops due to rise in fluid pressure and damage to the optic nerve of the eye. It is associated with eye’s drainage system which if block and does not work properly, leading to an accumulation of fluid inside the eyes. Glaucoma is an asymptomatic condition that slowly progresses and obliviously leads to permanent vision loss.

What is glaucoma?

To understand glaucoma, one must know about eye’s fluid-production and drainage system. The human eyes have ciliary bodies right behind the iris. The ciliary bodies continuously produce aqueous fluid to fill the hollow space between cornea and iris of the eyes. This fluid is essential for proper functioning of eyes. Eyes also have a drainage system at the corners of the iris that drains out extra fluid from eyes and maintain the intra-ocular pressure or eye’s inner pressure. If drainage canals of eyes block due to certain reasons, the fluid starts accumulating inside the eyes and imbalance the eye’s inner pressure. Consequently, eye’s nerves and retina severely damage and lead to a condition called glaucoma which gradually cause permanent vision loss.

Causes

Our eyes drainage system can be affected by four main reasons depending on types of glaucoma.
Open-angle glaucoma occurs when tiny particles block our eye’s drainage canals and fluid buildup inside the eyes damage eye nerves and retina.

Closed-angle glaucoma occurs when angle between the cornea and the iris is small enough, it suddenly blocks the drainage canals and the fluid buildup inside the eyes damage eye nerves and retina. The closed-angle glaucoma is very painful with severe headache that require immediate treatment.

Normal tension glaucoma is also called low-tension or normal pressure glaucoma. It occurs even when the pressure inside the eye is under the normal range, 10-21mmHg. It is still unknown what causes normal tension glaucoma, but certain risks that scientists relate with this glaucoma include:
• Low blood pressure causes an insufficient blood supply to the optic nerve.
• Family history of glaucoma.
• Defects in mitochondria, and
• Weaken collagen fragile the nerve fibers.
Congenital glaucoma occurs in newborn babies during fetal development. It is characterized by the abnormal growth of the eye's drainage canals.

Symptoms

Common symptoms of glaucoma are headache, eye pain and vision loss.

Diagnosis

Glaucoma is an asymptomatic condition which is hard to detect; however, few tests such as visual acuity and field tests, tonometry, slit-lamp exam, optical coherence tomography (OCT), and gonioscopy are used.

How Parathyroid Gland Surgery Is Performed?

Parathyroidectomy is a surgical procedure to remove abnormal or diseased parathyroid glands. Humans have four oval-shaped parathyroid glands located behind the thyroid glands in the neck. Parathyroid glands produce special hormone called parathyroid hormone or PTH. This hormone regulates calcium level in the blood which is essential for proper functioning of body organs.

Why is parathyroidectomy performed?

Due to certain reasons, if our parathyroid glands overstimulate or overact, they develop into a condition known as hyperparathyroidism. In this condition, abnormal parathyroid glands produce and release high levels of PTH hormone even when it is not required. Therefore, hyperparathyroidism must be treated immediately by removing diseased or abnormal parathyroid gland and this can only be done surgically through a procedure called parathyroidectomy.

Why does hyperparathyroidism occur?

Hyperparathyroidism occurs due to tumor growth in the parathyroid glands or vitamin D deficiency in our body. Due to unknown reasons, if tumor grows in our parathyroid glands, it causes abnormal division and proliferation of parathyroid cells and results in enlargement of parathyroid glands. The enlarged parathyroid glands will overact and produce abnormally high levels of PTH hormone. This leads to the development of hyperparathyroidism. In other case, because vitamin D is responsible for absorption of calcium from intestine into the blood. Therefore, if vitamin D deficiency occurs in our body, calcium will not be absorbed into the blood and blood calcium level falls critically. This stimulates parathyroid glands which continuously grow and become enlarged to cope the low blood calcium level. As a result, hyperparathyroidism occurs which needs immediate removal of abnormal parathyroid glands through a surgical procedure known as parathyroidectomy.

How is parathyroidectomy performed?

Before surgery, doctors examine test reports of few blood tests including complete blood count or CBC etc.,
During the procedure, patients typically receive general anesthesia to become unconscious during the surgery. Once the patient completely sedate, surgeons make about half-inch incisions in two sides of the neck and use special retractors to approach parathyroid glands without damaging the nearby tissues. Through one incision, an endoscope with camera in front is inserted to visualize the diseased parathyroid glands which are then removed with the help of instruments inserted through the second incision. Once, surgeon ensures that all the diseased parathyroid glands are removed, he sutures the subcutaneous layer and fix the top skin layer using surgical glue.
Parathyroidectomy is mostly applied in outpatient settings means that patients do not have to stay overnight in the hospital, even they are discharged within two to three hours of surgery.
