EU-Long-Term Residence Application required documents:
1. Application form for EU long-term resident permit, duly completed and signed.
2. Photographs — 4 recent passport-style photos (35×45 mm), colour, clear, recent (taken within last 6 months).
3. Valid travel document + photocopies — your current passport (plus copies of all pages), plus earlier passports showing stays & visas if relevant.
4. Documentation of legal and continuous stay in Poland over at least 5 years — e.g. previous residence cards/decisions, passport stamps/visas, travel history.
5. Proof of stable and regular income for last 3 years (or 2 years if special case) — employment contracts, annexes, PIT-37, PIT-11 returns, or other documented income (e.g. from company, business).
6. Proof of health insurance — e.g. confirmation of public health insurance (ZUS or equivalent) or other acceptable insurance.
7. Proof of accommodation / housing — valid rental agreement or other legal title to residence.
8. Polish language proficiency proof — certificate of Polish language at least B1, or diploma/certificate from Polish-language school/university.
9. Statement of trips abroad over past 5 years — listing departures and returns (especially relevant to show continuity).
10. Any additional documents needed to support your case — e.g. if income comes from business or position: company resolution appointing you, documents proving company income, tax books, etc.
11. Proof of payment of application fee (stamp duty).
